    One of the most pressing issues visible in the Utena and Grodno counties in recent decades is population decline. Low birth rate and negative net migration, premature mortality, lead to population decline, also increase the proportion of older people within the total population. Aging population and polimorbidity of elderly people increases demand of healthcare services. Accessibility and quality of health care services, provided in the counties are insufficient and the needs of elderly are not met: there is no possibility to get necessary specialized quite simple outpatient and day surgery healthcare services closer to their home (at county level), therefore elderly have to go to the biggest cities and pay for the services and/or for accommodation, go several times for consultations and it is usually difficult for the socio-economic reasons. There is also long waiting time to receive these services. All of this lead delayed health cases, which require more complex treatment and then consequences are less good. The main goal of the project is to increase quality of life of elderly people in Utena and Grodno counties by increasing accessibility and/or improving the quality of specialized (diabetes, ophthalmology and urology) outpatient and day surgery services in the institutions. During Project implementation by coordinating integrated actions positive results are expected. The health literacy and awareness of target groups will be increased, the services will be developed in order to ensure better access (to a maximum possible extent) to a high-quality public specialized day surgery and outpatient healthcare services, health professionals will share and improve their knowledge. Interventions envisaged in the Project will help to improve services relevant for indicated vulnerable group, to create preconditions for longer and healthier living for this population group, lower risks of disability or premature deaths, social exclusion and poverty.
