Чрез реализиране на дейностите по настоящия проект ще се предоставят интегрирани услуги за хората с невъзможност за самообслужване и хората с увреждане, като ще се съчетаят комплексни действия в посока осигуряване на дългосрочна грижа, вкл. социални и здравни услуги в общността или в домашна среда. Чрез предоставяне на услугите „личен асистент” , "социален асистент" и "домашен помощник"ще се осигури превенция на институционализацията на целевите групи, ще се подобри качеството им на живот и ще се подпомогнат възможностите им за независимост и за социално включване. Ще се разшири обхвата на ползваните услуги, като се осигури достъп не само до социални, но и до здравни и др. вид услуги предоставяни в общността.Едновременно с това ще се осигури заетост на безработни лица в трудоспособна възраст, а чрез проведеното обучение ще се повиши тяхната компетентност. Through the implementation of the activities under this project, integrated services will be provided to people with inability to self-service and people with disabilities, combining complex actions to provide long-term care, including social and health services in the community or in the home environment. The provision of the services of “personal assistant”, “social assistant” and “home assistant” will ensure the prevention of institutionalisation of the target groups, improve their quality of life and support their opportunities for independence and social inclusion. The scope of the services used will be expanded, ensuring access not only to social, but also to health and other services provided in the community. Simultaneously, it will provide employment for unemployed people of working age, and through training will increase their competence. (English)

Grâce à la mise en œuvre des activités menées dans le cadre de ce projet, des services intégrés seront fournis aux personnes incapables de se rendre en libre-service et aux personnes handicapées, en combinant des mesures complexes pour fournir des soins de longue durée, y compris des services sociaux et de santé dans la collectivité ou à domicile. La fourniture des services d’«assistant personnel», d’«assistant social» et d’«assistant à domicile» permettra de prévenir l’institutionnalisation des groupes cibles, d’améliorer leur qualité de vie et de soutenir leurs possibilités d’indépendance et d’inclusion sociale. Le champ d’application des services utilisés sera élargi, ce qui garantira l’accès non seulement aux services sociaux, mais aussi aux services de santé et aux autres services offerts dans la communauté; parallèlement, il fournira des emplois aux chômeurs en âge de travailler et, grâce à la formation, il augmentera leurs compétences. Durch die Durchführung der Maßnahmen im Rahmen dieses Projekts werden integrierte Dienstleistungen für Menschen mit Selbstbedienungsunfähigkeit und Menschen mit Behinderungen bereitgestellt, die komplexe Maßnahmen zur Langzeitpflege, einschließlich Sozial- und Gesundheitsdienstleistungen in der Gemeinschaft oder im häuslichen Umfeld, kombinieren. Die Erbringung der Dienstleistungen von „persönlichen Assistenten“, „Sozialassistenten“ und „Heimatassistenten“ wird die Prävention der Institutionalisierung der Zielgruppen gewährleisten, ihre Lebensqualität verbessern und ihre Möglichkeiten für Unabhängigkeit und soziale Inklusion unterstützen. Der Umfang der in Anspruch genommenen Dienstleistungen wird erweitert, um den Zugang nicht nur zu sozialen, sondern auch zu Gesundheits- und sonstigen Dienstleistungen in der Gemeinschaft zu gewährleisten. Door de uitvoering van de activiteiten in het kader van dit project zullen geïntegreerde diensten worden verleend aan mensen met onvermogen tot zelfbediening en mensen met een handicap, waarbij complexe acties worden gecombineerd om langdurige zorg te verlenen, met inbegrip van sociale en gezondheidsdiensten in de gemeenschap of in de thuisomgeving. Het verlenen van de diensten van „persoonlijk assistent”, „sociale assistent” en „thuisassistent” zal de institutionalisering van de doelgroepen voorkomen, hun levenskwaliteit verbeteren en hun kansen op onafhankelijkheid en sociale integratie ondersteunen. Het toepassingsgebied van de gebruikte diensten zal worden uitgebreid, zodat niet alleen toegang wordt geboden tot sociale, maar ook tot gezondheidszorg en andere diensten die in de gemeenschap worden verleend. Tegelijkertijd zal het werk bieden aan werklozen in de werkende leeftijd en door middel van opleiding zal hun competentie worden vergroot. (Dutch)

Attraverso l'attuazione delle attività nell'ambito di questo progetto, saranno forniti servizi integrati alle persone con incapacità al self-service e alle persone con disabilità, combinando azioni complesse per fornire assistenza a lungo termine, compresi i servizi sociali e sanitari nella comunità o nell'ambiente domestico. La fornitura dei servizi di "assistente personale", "assistente sociale" e "assistente domestico" garantirà la prevenzione dell'istituzionalizzazione dei gruppi destinatari, migliorerà la loro qualità di vita e sosterrà le loro opportunità di indipendenza e inclusione sociale. La portata dei servizi utilizzati sarà ampliata, garantendo l'accesso non solo ai servizi sociali, ma anche ai servizi sanitari e ad altri servizi forniti nella comunità, fornendo simultaneamente un'occupazione ai disoccupati in età lavorativa e, attraverso la formazione, aumenterà le loro competenze. Mediante la ejecución de las actividades en el marco de este proyecto, se prestarán servicios integrados a las personas con incapacidad para el autoservicio y a las personas con discapacidad, combinando acciones complejas para prestar cuidados de larga duración, incluidos los servicios sociales y de salud en la comunidad o en el hogar. La prestación de los servicios de «asistente personal», «asistente social» y «asistente a domicilio» garantizará la prevención de la institucionalización de los grupos destinatarios, mejorará su calidad de vida y apoyará sus oportunidades de independencia e inclusión social. El alcance de los servicios utilizados se ampliará, garantizando el acceso no solo a los servicios sociales, sino también a los servicios de salud y otros servicios prestados en la comunidad; simultáneamente, proporcionará empleo a los desempleados en edad de trabajar y, a través de la formación, aumentará su competencia.
