Проектното предложение е насочено към подобряване на качеството на живот и възможностите за социално включване на хората с увреждания и възрастните хора в община Своге, чрез осигуряване на мрежа от услуги в домашна среда и изграждане на подходящ капацитет за предоставянето им чрез въвеждане на модел за патронажни грижи за възрастни хора и лица с увреждания, вкл. с хронични заболявания и трайни увреждания за осигуряване на почасови мобилни интегрирани здравно-социални услуги в техните домове. Реализирането на проекта ще осъществи чрез възлагане на дейността по патронажна грижа на Звеното към община Своге „Център за почасово предоставяне на услуги за социално включване в общността и в домашна среда“. Центърът е създаден по проект „Нова възможност за социална подкрепа и независим живот на хората в неравностойно положение в Община Своге“ по Оперативна програма „Развитие на човешките ресурси“. Предоставянето на интегрирани здравно-социални услуги се предвижда да бъде за период от 12 месеца за минимум 88 потребители. Чрез този модел ще се структурира и стандартизира предоставянето на здравно-социални патронажни грижи на база разработената от МЗ програма за специфично обучение на медицинските специалисти и специалистите в областта на социалните услуги, които ще предоставят патронажните грижи. Чрез разработената от МЗ програма/методика за предоставяне на патронажната грижа, ще се създадат единни указания на предоставящите услугите специалисти на територията на общината. Предвидените проектни дейности са:- Подбор на потребители и персонал на услугите. Staff training and supervision.- Материално - техническо обезпечаване на услугите.- Предоставяне на почасови мобилни интегрирани здравно-социални услуги за нуждаещи се лица с увреждания и възрастни хора.Изпълнението на дейностите ще допринесе за предоставяне на нов вид интегрирани здравно-социални услуги за дългосрочна грижа в общността, с необходимото качество и достъпност за хората с уреждания и възрастните хора. The project proposal aims at improving the quality of life and opportunities for social inclusion of people with disabilities and the elderly in the municipality of Svoge, by providing a network of services in the home environment and building appropriate capacity for their provision by introducing a model for patronage care for elderly people and persons with disabilities, including chronic diseases and permanent disabilities to provide hourly mobile integrated health and social services in their homes. The implementation of the project will be carried out by entrusting the patronage care activity to the Svoge Municipality “Centre for hourly provision of services for social inclusion in the community and in the home environment”. The Center was created under the project “New opportunity for social support and independent living of disadvantaged people in Svoge Municipality” under the Operational Programme “Human Resources Development”. The provision of integrated health-social services is foreseen for a period of 12 months for a minimum of 88 users. This model will structure and standardise the provision of health-social patronage care on the basis of the specific training programme developed by the Ministry of Health for health professionals and social services specialists who will provide patronage care. The program/methodology for providing patronage care developed by the Ministry of Health will create uniform guidelines for the specialists providing the services on the territory of the municipality. The envisaged project activities are:- Selection of users and staff of the services. Staff training and supervision.- Material — technical provision of services.- Provision of hourly mobile integrated health-social services for deprived persons with disabilities and elderly people.The implementation of the activities will contribute to the provision of a new type of integrated health-social services for long-term care in the community, with the necessary quality and accessibility for the people with arrangements and the elderly.
