Celem głównym proj jest aktywizacja społeczno - zawodowa 80 osób (63K, 17 M) zagrożonych ubóstwem lub wykluczeniem społecznym, zamieszkujących w myśl KC woj. zachodniopomorskie w Gminie Goleniów, poprzez realizację indywidualnej ścieżki reintegracji do 30.06.2019. W proj założono osiągnięcie wskaźników: • Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych wsparciem– 80 • Liczba osób z niepełnosprawnościami objętych wsparciem - 17 • Liczba osób zagrożonych ubóstwem lub wykluczeniem społ., które uzyskały kwalifikacje– 25 (min. 31%). • Liczba osób zagrożonych ubóstwem lub wykl. społ. poszukujących pracy – 20 (min. 25%). • Liczba osób zagrożonych ubóstwem lub wykluczeniem społ. pracujących – 14 (min.15%). • Liczba os. zagrożonych ubóstwem lub wykl. społ., które nabyły kompetencje– 16. Grupę docelową stanowi 80 osób (63 K, 17 M) zamieszkałych w Gminie Goleniów, zagrożonych ubóstwem lub wykluczeniem społ. Będą to osoby korzystające ze świadczeń z pomocy społ. zg. z ustawą z 12.03.2004 r. o pomocy społ. lub kwalifikujące się do objęcia wsparciem pomocy społ. Do proj zostaną przyjęte osoby spełniające min. 1 przesłankę zagrożenia ubóstwem lub wykluczeniem społecznym: - os bierne ZAW - 43 UP (35 K, 8 M), - os bezrobotne – 37 UP (28 K, 9 M) - os niepełnosprawne - 17 UP (10K), w tym 8 o znacznym stopniu niepełnospr., os z niepełnospr. intelekt., os z niepełnospr. sprzężonymi. W ramach projektu zostaną zrealizowane takie zadania, jak: 1. Aktywizacja społeczna 2. Działania animatora społeczności lokalnej 3. Program Aktywności Lokalnej 4. Aktywizacja zdrowotna 5. Aktywizacja edukacyjna 6. Aktywizacja zawodowa. (Polish)

The main objective of the project is to activate the socio-occupational activity of 80 people (63K, 17 M) at risk of poverty or social exclusion, living in the West Pomeranian Voivodeship in the Goleniów Municipality, by implementing an individual path of reintegration up to 30.06.2019. The project assumes the achievement of indicators: • Number of people at risk of poverty or social exclusion supported – 80 • Number of persons with disabilities supported – 17 • Number of people at risk of poverty or social exclusion who have received qualifications – 25 (min. 31 %). • Number of people at risk of poverty or job seekers – 20 (min. 25 %). • Number of people at risk of poverty or social exclusion in employment – 14 (min.15 %). • The number of people at risk of poverty or outlaws who have acquired competences – 16. The target group consists of 80 people (63 K, 17 M) living in the Goleni District, at risk of poverty or social exclusion. These will be persons benefiting from the social aid benefits in accordance with the Act of 12.03.2004 on social aid or eligible for community aid. Persons fulfilling at least 1 conditions of at risk of poverty or social exclusion will be admitted to the project: — passive wass ZAW – 43 UP (35 K, 8 M), – wasp unemployed – 37 UP (28 K, 9 M) – wasp disabled – 17 UP (10K), including 8 with a significant degree of disability, wasp with an intellect. The project will carry out tasks such as: 1. Social Activity 2. Activities of the local community animator 3. Local Activity Programme 4. Health Activation 5. Educational Activity 6. L’objectif principal du projet est l’activation socioprofessionnelle de 80 personnes (63K, 17 M) menacées de pauvreté ou d’exclusion sociale, vivant conformément au KC de la voïvodie de Poméranie occidentale dans la municipalité de Goleniów, grâce à la mise en œuvre d’une voie de réinsertion individuelle jusqu’au 30.6.2019. Le projet suppose la réalisation d’indicateurs: • Nombre de personnes menacées de pauvreté ou d’exclusion sociale couvertes — 80 • Nombre de personnes handicapées bénéficiant d’un soutien — 17 • Nombre de personnes exposées au risque de pauvreté ou d’exclusion sociale ayant obtenu une qualification — 25 (minimum 31 %). • Le nombre de personnes exposées au risque de pauvreté ou de faute professionnelle — 20 (min. 25 %). • Le nombre de personnes menacées de pauvreté ou d’exclusion dans l’emploi — 14 (min.15 %). • Le nombre de personnes exposées au risque de pauvreté ou de faute professionnelle ayant acquis des compétences — 16. Le groupe cible est de 80 personnes (63 K, 17 M) vivant dans la municipalité de Goleniów, menacées de pauvreté ou d’exclusion sociale. Il s’agira de personnes bénéficiant de l’aide sociale conformément à la loi du 12 mars 2004 sur l’aide sociale ou pouvant bénéficier de l’aide sociale. Les personnes remplissant au moins une condition de risque de pauvreté ou d’exclusion sociale seront admises au proj: — passive OS ZAW — 43 UP (35 K, 8 M), — chômeurs — 37 UP (28 K, 9 M) — Personnes handicapées — 17 UP (10K), dont 8 avec un degré significatif d’incomplète, cégep avec une intellect incomplète. Le projet exécutera des tâches telles que: 1. Activation sociale 2. Activités de l’animateur communautaire local 3. Programme d’activités locales 4. Activation sanitaire 5.
