This project assumes the implementation by the West Pomeranian Centre of Oncology of the Regional Health Programme of the West Pomeranian Voivodeship entitled “Medical rehabilitation of oncological patients for the period 2020-2022”. The main objective of the project is to support the rehabilitation of 500 people (320K and 180M) from the West Pomeranian Voivodeship at the age of work for patients who are in the course of or after completion of radical oncological treatment, on the basis of inclusion criteria and on the basis of the priority of application. As a result, at least 60 % of these people will maintain their current employment or find a job. In order to recruit Programme participants, the Applicant plans to carry out a wide-ranging information and promotion campaign addressed to residents of the West Pomeranian Voivodeship. Subsequently, as a result of the received applications, the qualification of the participants for the Programme will be carried out by qualified medical staff. The next stage will be conducting medical rehabilitation of qualified patients (500 people) and providing psychological support and organisation of educational classes with physiology and nutritionist. In order to facilitate and improve accessibility to the Program, the Applicant plans to reimburse the costs of commuting to rehabilitation camps, as well as to purchase the necessary rehabilitation equipment. In addition, the project also plans to train 50 people (40K and 10M) of medical personnel from the West Pomeranian Voivodeship on the principles of medical care, as well as therapeutic and rehabilitation procedures in patients after undergoing oncological treatment. The project is planned for the period May 2021 — December 2022. Throughout the implementation of the investment, the Applicant will regularly monitor the results of the implementation of the Programme and the level of achievement of the assumed values concerning the output and result indicators of the project. Niniejszy projekt zakłada realizację przez Zachodniopomorskie Centrum Onkologii Regionalnego Programu Zdrowotnego Województwa Zachodniopomorskiego pn. "Rehabilitacja lecznicza pacjentów onkologicznych na lata 2020 -2022". Głównym założeniem projektu jest objęcie wsparciem w zakresie rehabilitacji 500 osób (320K i 180M) z woj. zachodniopomorskiego w wieku aktywności zawodowej dla pacj. będących w trakcie lub po zakończ. radykalnym leczeniu onkologicznym, na podstawie kryteriów włączenia i według kolejności zgłoszeń. W wyniku tego co najmniej 60% z tych osób utrzyma swoje aktualne zatrudnienie bądź znajdzie pracę. W celu rekrutacji uczestników Programu, Wnioskodawca planuje przeprowadzić szeroko zakrojoną i skierowaną do mieszkańców woj. zachodniopomorskiego kampanię informacyjno-promocyjną. Następnie w wyniku otrzymanych zgłoszeń przeprowadzona zostanie przez wykwalifikowaną kadrę medyczną kwalifikacja uczestników do Programu. Kolejnym etapem będzie przeprowadzenie rehabilitacji medycznej zakwalifikowanych pacjentów (500 osób) oraz udzielenie wsparcia psychologicznego oraz organizację zajęć edukacyjnych z fizjologiem i dietetykiem. W celu ułatwienia i poprawy dostępności do Programu, Wnioskodawca planuje zwracać koszty dojazdów na turnusy rehabilitacyjne, jak również zakupić niezbędny sprzęt rehabilitacyjny. Ponadto w ramach projektu zaplanowano też przeszkolenie 50 osób (40K i 10M) personelu medycznego z woj. zachodniopomorskiego w zakresie zasad opieki medycznej, a także postępowania leczniczego i rehabilitacyjnego u pacjentów po przebytym leczeniu onkologicznym. Realizację projektu przewidziano na okres maj 2021 r. – grudzień 2022 roku. Przez cały czas realizacji inwestycji Wnioskodawca będzie dokonywał regularnego monitorowania efektów realizacji Programu oraz poziomu osiągnięcia założonych wartości dotyczących wskaźników produktu i rezultatu projektu.
