The present project “Multi-layered approaches to cross-border communication and cooperation in emergency medicine” (gekom) originated from the experience with previous Interreg projects, the identified needs and requirements of the German-Polish border area. With its three pillars “Network”, “Communication and Language” and “cross-border simulation training”, the project serves the sustainable improvement of emergency medical patient care in the assisted area. In the first work package, the existing care structures and chains between the individual organisational units in emergency medicine (emergency rooms, emergency services, GP practices, etc.) are analysed. The experts will identify factors that inhibit or positively reinforce patients’ cross-border emergency care. Experience from the already completed projects will be transferred in order to achieve positive effects for the participating institutions. Symposia are planned to enable the networking of the professional experts of the projects, the mutual presentation of the project objectives and the cooperation between existing projects and initiatives and the associated use of synergy effects. This will also make the issue of emergency medical care available to the general public in the border area and inform citizens and guests about the activities and raise awareness of the projects, their sponsors but also the challenges in the region. Language training for all employees of emergency medical institutions in the assisted area is of course not possible as part of a project. In addition, personnel fluctuations in emergency medical professions are not uncommon. Therefore, the development, implementation and evaluation of German-Polish communication aids to support communication with the patient as well as with other actors in the rescue chain such as rescue workers, hospital staff, emergency services, etc. form the second work package of this project. These communication aids also support the cross-border rescue agreements in the cooperation agreements currently concluded along the border between the voivodships and districts and will allow employees to carry out a high-quality cross-border rescue in the future. The third work package of the project is generated by the linguistic and professional challenges in the border area. The employees in emergency services, hospitals, GP and emergency service practices as well as other associated institutions have the opportunity to participate in the innovative emergency medical language tandem training circle, which is explicitly tailored to the needs and requirements of personnel in cross-border emergency medicine. The multi-day modules that combine communication and practice are completed with certificates. These certificates enable participants to demonstrate their acquired skills and also serve as motivation for sustainable participation in the training circle. The aim is that at the end of the training circle, the participating employees are able to interview a foreign-language patient about their symptoms and pre-existing diseases, explain them necessary measures and then provide the patient with optimal care. Our goal is to promote the sustainability of cross-border rescue in practice and our overarching vision is to incorporate the knowledge into regular operation and portability to other border regions.     Das vorliegende Projekt „Mehrschichtige Ansätze zur grenzüberschreitenden Kommunikation und Kooperation in der Notfallmedizin“(GeKoM) ist aus den Erfahrungen mit vorherigen Interreg-Projekten, den identifizierten Bedürfnissen und Anforderungen des deutsch-polnischen Grenzgebiets entstanden. Das Projekt dient mit seinen drei Säulen „Vernetzung“, „Kommunikation und Sprache“ sowie „grenzüberschreitendes Simulationstraining“ der nachhaltigen Verbesserung der notfallmedizinischen Patientenversorgung im Fördergebiet. Im ersten Arbeitspaket werden die bestehenden Versorgungsstrukturen und –ketten zwischen den einzelnen Organisationseinheiten in der Notfallmedizin (Notaufnahmen, Rettungsdienste, hausärztliche Praxen, etc.) analysiert. Von den Experten werden Faktoren herausgearbeitet, die die grenzüberschreitende Notfallversorgung von Patienten hemmen oder positiv verstärken. Dabei werden Erfahrungen aus den bereits abgeschlossenen Projekten transferiert, um positive Wirkungen für die beteiligten Institutionen zu erreichen. Um die Vernetzung der professionellen Experten der Projekte, die gegenseitige Vorstellung der Projektziele sowie der Kooperation der bereits existierenden Projekte und Initiativen untereinander und der damit verbundenen Nutzung von Synergieeffekten zu ermöglichen, werden Symposien geplant. Damit wird das Thema der notfallmedizinischen Versorgung im Grenzgebiet auch der breiten Öffentlichkeit zugänglich gemacht und die Bürger*innen sowie Gäste werden über die Aktivitäten informiert und für die Projekte, ihre Fördergeber aber auch die Herausforderungen in der Region sensibilisiert. Eine sprachliche Schulung aller Mitarbeitenden der notfallmedizinischen Institutionen im Fördergebiet ist im Rahmen eines Projektes selbstverständlich nicht möglich. Zudem sind Personalfluktuationen in notfallmedizinischen Berufen keine Seltenheit. Daher bilden die Entwicklung, Implementierung und Evaluation von deutsch-polnischen Kommunikationshilfen zur Unterstützung der Kommunikation mit dem Patienten sowie mit anderen Akteuren der Rettungskette wie Rettungsdienstmitarbeitende, Krankenhauspersonal, Notdiensten, etc. das zweite Arbeitspaket dieses Projektes. Diese Kommunikationshilfen unterstützen ebenfalls die Vereinbarungen zur grenzüberschreitenden Rettung in den Kooperationsverträgen, die aktuell entlang der Grenze zwischen den Wojewodschaften und Landkreisen geschlossen werden und werden es den Mitarbeitenden erlauben, eine qualitativ hochwertige grenzüberschreitende Rettung in der Zukunft durchzuführen. Das dritte Arbeitspaket des Projektes wird durch die sprachlichen und beruflichen Herausforderungen im Grenzbereich erzeugt. Die Mitarbeitenden in Rettungsdiensten, Krankenhäusern, Hausarzt- und Notdienstpraxen sowie weitere assoziierte Institutionen haben die Möglichkeit, an dem innovativen notfallmedizinischen Sprach-Tandem-Fortbildungszirkel teilzunehmen, der explizit auf die Bedürfnisse und Anforderungen von Personal in der grenzüberschreitenden Notfallmedizin zugeschnitten ist. Die mehrtägigen Module, die Kommunikation und Praxis verbinden, werden mit Zertifikaten abgeschlossen. Diese Zertifikate ermöglichen es den Teilnehmenden, ihre erworbenen Fähigkeiten nachzuweisen und dienen zusätzlich als Motivation für eine nachhaltige Teilnahme am Fortbildungszirkel. Ziel ist es, dass am Ende des Fortbildungszirkels die teilnehmenden Mitarbeitende in der Lage sind, einen fremdsprachigen Patienten zu seinen Symptomen und Vorerkrankungen zu befragen, ihm notwendige Maßnahmen zu erklären und im Anschluss den Patienten optimal zu versorgen. Unser Ziel ist es, die Nachhaltigkeit der grenzüberschreitenden Rettung in der Praxis dauerhaft zu fördern und unsere übergeordnete Vision ist die Übernahme der Erkenntnisse in den Regelbetrieb und die Übertragbarkeit auf weitere Grenzregionen.
