Project “For Independent Life” is aimed at improving the quality of life and overcoming the consequences of social exclusion and poverty through access to services for social inclusion of people with disabilities and people over 65 years old, incapable of self-service by the municipality of Simitli. For this purpose, conditions for long-term care in the community or in a home environment will be created by organising a Center for hourly provision of services for social inclusion. We foresee to be located in a building, municipal property and within the project the designated premises will be renovated, equipped and furnished. For the provision of integrated services, a team of 27 trained domestic assistants, personal and social assistants, social worker, psychologist, nurse and rehabilitator will be formed. The staff providing social services will be provided psychological support and supervision. Detailed rules and requirements for the functioning of the Centre and the provision of services will be developed. Complex services, motivational and psychological support within 20 months will receive 70 persons with disabilities and people over 65 years old with restrictions or in the inability to self-service in the community or in a home environment, selected according to their individual needs.The project covers all 18 settlements of the municipality, and for the users of the more remote villages, the hard-to-mobile and disabled people will be provided a specialised vehicle for their transport to the Centre.The main result of the project will be improved access to hourly services for social inclusion in the community or in a home environment for 70 people from the municipality, guaranteed by rules and procedures for selection, inclusion and assessment of users’ needs, provision of services, motivated and qualified team for work, modern material environment and transport access.The project will be implemented 25 months by a management team, including a manager, coordinator and accountant. Проект „За независим живот“ е насочен към подобряване качеството на живота и преодоляване последиците от социалното изключване и бедността чрез достъп до услуги за социално включване на хората с увреждания и хората над 65 г. в невъзможност за самообслужване от община Симитли.За целта ще бъдат създадени условия за дългосрочна грижа в общността или в домашна среда чрез организиране на Център за почасово предоставяне на услуги за социално включване.Предвиждаме той да бъде разположен в сграда, общинска собственост и в рамките на проекта отредените помещения ще бъдат ремонтирани, оборудвани и обзаведени. За предоставянето на интегрирани услуги ще бъде сформиран екип от 27 обучени домашни помощници, лични и социални асистенти, социален работник, психолог, медицинска сестра и рехабилитатор.На персонала, предоставящ социални услуги ще бъде оказвана психологическа подкрепа и супервизия.Ще бъдат разработени детайлни правила и изисквания за функционирането на Центъра и предоставянето на услуги. Комплексни услуги, мотивационна и психологическа подкрепа в рамките на 20 месеца ще получат 70 лица с увреждания и хора над 65 г. с ограничения или в невъзможност за самообслужване в общността или в домашна среда, подбрани според индивидуалните им потребности.Проектът обхваща всички 18 населени места на общината, като за потребителите от по-отдалечените села, трудно подвижните и за лицата с увреждания ще бъде осигурено специализирано превозно средство за превоза им до Центъра.Основният резултат от изпълнението на проекта ще бъде подобрен достъп до почасови услуги за социално включване в общността или в домашна среда за 70 лица от общината, гарантиран чрез правила и процедури за подбор, включване и оценка на потребностите на потребителите, предоставяне на услугите, мотивиран и квалифициран екип за работа, съвременна материална среда и осигурен транспортен достъп.Проектът ще се изпълнява 25 месеца от сформиран екипа за управление, включващ ръководител, координатор и счетоводител.
