Ziel des Vorhabens ist die sozialpädagogische Begleitung von Schüler_innen mit besonderem sozialpädagogischen Unterstützungsbedarf zur Kompetenzförderung bei Vorliegen von individuellen Beeinträchtigungen oder sozialen Benachteiligungen mit dem Schwerpunkt dr Einzelfallhilfe. Für eine zielorientierte Einzelfallarbeit werden individuelle Förderpläne erstellt. Die Förderpläne sind für die Teilnehmer_innen verständlich, überschaubar und konkret. Die in den Förderplänen formulierten Ziele werden in regelmäßigen Abständen gemeinsam mit den Teilnehmer_innen in den Blickgenommen und hinsichtlich der Zielerreichung reflketiert. Die detailierte Beschreibung des Vorhabens ist im Konzept niedergelgt. (German)
