For the implementation of the project, there will be extensive infrastructure and technical facilities in the form of a second level, which is currently under way in the framework of a project for the complementary revitalisation of the former monastery of Eltanek, Elżbieta, in Gorzów Silesia, together with equipment adapted to transport persons with a disabled vehicle and owned and acquired as part of the design of a therapeutic equipment. In the first phase of the project, recruitment of staff with qualifications suitable for the provision of care, activation and rehabilitation services for the specific group of elderly patients with disabilities, resulting not only from diseases such as stroke or injury, but also naturally occurring as a result of progressive ageing processes, will be carried out. In parallel, project participants will be recruited in accordance with the pre-established rules of procedure. At the same time, small equipment will be purchased — the main expenditure will be paid in a complementary project. At the same time, 20 persons will be provided with care, and a member of the municipality of Gorzów Śląski and District of Oleski, but also, if necessary and to the extent to which the possibilities are available, also from the whole Opolszczyzny. The project provides funding for the provision of care facilities in okr 3 years, from June ‘19 to May’ 22; in subsequent periods, the costs of the benefits will be covered by the Applicant’s own resources and by the targeted subsidy. Projekt przewiduje podjęcie działań na rzecz rozwoju zdeinstytucjonalizowanych form opieki medycznej nad osobami starszymi, w tym z niepełnosprawnościami, poprzez utworzenie nowych miejsc świadczenia dziennych form usług opiekuńczych, zwiększenie dostępu do usług poprzez zapewnienie niezbędnego transportu oraz rozwój działań wpływających na poprawę stanu zdrowia tych osób dzięki zapewnieniu stałego dostępu do pozastandardowych świadczeń medycznych w postaci rehabilitacji i krioterapii. Dla realizacji projektu zaangażowane przez Wnioskodawcę zostanie szerokie zaplecze infrastrukturalne i techniczne w postaci drugiej kondygnacji podlegającego obecnie w ramach projektu komplementarnego rewitalizacji dawnego klasztoru Elżbietanek w Gorzowie Śląskim wraz z wyposażeniem, odpowiednio przystosowanego do przewozu osób niepełnosprawnych pojazdu oraz posiadanego i nabytego w ramach projektu wyposażenia terapeutycznego. W pierwszym etapie realizacji projektu dokonana zostanie rekrutacja personelu o kwalifikacjach odpowiednich dla udzielania świadczeń opiekuńczych, aktywizacyjnych i rehabilitacyjnych dla specyficznej grupy pacjentów w starszym wieku, obarczonych niepełnosprawnościami wynikającymi nie tylko z przebiegu chorób takich jak udar czy urazów, ale również występującymi naturalnie w wyniku postępujących procesów starzenia. Równolegle prowadzony będzie nabór uczestników projektu prowadzony zgodnie z opracowanym wcześniej regulaminem. W tym samym czasie zakupiony zostanie drobne wyposażenie – główne wydatki poniesione zostaną w projekcie komplementarnym. Świadczenia opiekuńcze udzielane będą równocześnie 20 osobom, gł mieszk gminy Gorzów Śląski i powiatu oleskiego, ale także, w razie potrzeby i w miarę posiadanych możliwości, również z całej Opolszczyzny. Projekt przewiduje finansowanie miejsc świadczenia usług opiekuńczych w okr 3 lat, od czerwca '19 do maja '22; w kolejnych okresach koszty świadczeń pokrywane będą ze środków własnych Wnioskodawcy oraz dotacji celowy (Polish)
