    La circulación de sarampión, rubéola, parotiditis, varicela y B.pertussis es muy baja en España, pero el aumento de población inmigrante con distintas características epidemiológicas, el número de viajes internacionales y la creciente aparición de grupos antivacunas en algunos sectores de la sociedad, hacen que la situación sea ideal para la aparición de brotes epidémicos. Se llevará a cabo un estudio retrospectivo aplicando un modelo espacio-temporal al Sistema Nacional de Vigilancia Epidemiológica para datos hospitalarios (Conjunto Mínimo Básico de Datos; CMBD) del Ministerio de Sanidad para identificar estos brotes en los últimos 17 años y ver si siguen algún patrón de comportamiento. Se seleccionarán mediante los códigos CIE-9 todas las altas hospitalarias debidas a las enfermedades a estudiar en cualquier posición diagnóstica correspondientes al período de estudio del 1 de enero de 1997 hasta 31 de diciembre de 2013 en los hospitales españoles. Para cada caso se recogerá información demográfica, clínica y geográfica. También se identificará prospectivamente brotes de sarampión, rubéola, parotiditis, varicela y tos ferina en España en el período de estudio mimetizando un sistema de vigilancia epidemiológica “near real time” aplicado a los datos hospitalarios disponibles. Los modelos espacio-temporales para la identificación de brotes epidémicos han sido utilizados con éxito en bases de datos administrativas, no sólo en la verificación de los brotes pasados o identificación de aquellos que nunca fueron declarados, sino también en la predicción de patrones futuros y vigilancia epidemológica “near real-time” y suponen una buena optimización de recursos en épocas de crisis.     The circulation of measles, rubella, mumps, chickenpox and B.pertussis is very low in Spain, but the increase in the immigrant population with different epidemiological characteristics, the number of international trips and the growing appearance of anti-vaccine groups in some sectors of society make the situation ideal for the emergence of epidemic outbreaks. A retrospective study will be carried out applying a spatial-temporal model to the National Epidemiological Surveillance System for hospital data (minimum basic data set; CMBD) of the Ministry of Health to identify these outbreaks in the last 17 years and see if they follow any pattern of behavior. All hospital discharges due to the diseases to be studied in any diagnostic position corresponding to the study period from 1 January 1997 to 31 December 2013 in Spanish hospitals will be selected using the CIE-9 codes. Demographic, clinical and geographical information shall be collected for each case. Outbreaks of measles, rubella, mumps, chickenpox and pertussis in Spain will also be prospectively identified in the study period by mimicking a “near real time” epidemiological surveillance system applied to the available hospital data. The time-space models for the identification of epidemic outbreaks have been used successfully in administrative databases, not only in the verification of past outbreaks or identification of those that were never declared, but also in the prediction of future patterns and epidemiological surveillance “near real-time” and represent a good optimisation of resources in times of crisis.
