The project concerns the creation of the necessary infrastructure — places to provide social services to people with intellectual disabilities in order to eliminate barriers to de-institutionalisation of the care system. The project includes the adaptation of 4 premises in Gdańsk (details in point D4), in order to implement the program of services in the form of assisted housing, respite care and self-reliance training of people in need of support in everyday functioning, together with appropriate retrofitting and management of the environment and the purchase of a passenger car for the transport of persons with disabilities. As a result of the implementation of the investment, the objective of the project will be achieved: increased availability of social services for people at risk of exclusion by increasing the number of places to provide social services, using environmental animation and volunteering instruments, as well as the development of infrastructure enabling comprehensive support for persons with disabilities and their families in Gdańsk. Projekt dotyczy utworzenia niezbędnej infrastruktury – miejsc świadczenia usług społecznych na rzecz osób z niepełnosprawnością intelektualną w celu likwidacji barier deinstytucjonalizacji systemu opieki. Przedsięwzięcie obejmuje adaptację 4 lokali w Gdańsku (szczegóły w pkt D4), w celu realizacji programu usług w formie mieszkań wspomaganych, opieki wytchnieniowej i treningu samodzielności osób potrzebujących wsparcia w codziennym funkcjonowaniu wraz z odpowiednim doposażeniem i zagospodarowaniem otoczenia oraz zakup samochodu osobowego do przewozu osób z niepełnosprawnościami. W wyniku realizacji inwestycji osiągnięty zostanie cel projektu: zwiększona dostępność usług społecznych dla osób zagrożonych wykluczeniem poprzez zwiększenie liczby miejsc świadczenia usług społecznych, przy wykorzystaniu instrumentów animacji środowiskowej i wolontariatu, a także rozwoju infrastruktury umożliwiającej realizację kompleksowego wsparcia na rzecz osób z niepełnosprawnością i ich rodzin w Gdańsku.
