This project proposal aims at improving the quality of life and expanding the opportunities for social inclusion of the elderly and people with disabilities in the municipality of Gabrovo, by providing patronage care in their homes.Specific objectives:- to improve the quality of life of consumers by providing specific social and health services appropriate to the needs;- to create a model of provision of patronage care to the elderly and people with disabilities;- to build capacity for providing quality patronage care.The main groups of the project are: — elderly people over 65 years of age with limitations or inability to self-service; — people with disabilities and their families; — employees of organisations and institutions involved in HEH processes/employees of social and health service providers. The following main activities will be carried out:1. Provision of conditions for the provision of the service “Patronage care”2. Regulating and securing the actual provision of the service “Patronage care”3. Provision of the social and health service “Patronage care”.The implementation of the project will last 18 months, starting with activities related to providing conditions for the provision of the service, including training of staff, regulation of the service for provision in Gabrovo municipality, selection of external contractors, etc. Provision of the socio-health service “Patronage care” will be implemented for a period of 12 months and will meet the needs of 224 users. Настоящото проектно предложение има за цел подобряване качеството на живот и разширяване възможностите за социално включване на възрастните и хората с увреждания в община Габрово, посредством предоставяне на патронажни грижи в техните домове.Специфични цели:- да се подобри качеството на живот на потребители, посредством предоставяне на специфични социално-здравни услуги, съответни на потребностите;- да се създаде модел на предоставяне на патронажни грижи на възрастни и хората с увреждания;- да се изгради капацитет за предоставяне на качествени патронажни грижи.Целеви групи на проекта са:- възрастни хора над 65 години с ограничения или в невъзможност за самообслужване; - хора с увреждания и техните семейства; - служители на организации и институции, ангажирани с процесите на ДЕИ/служители на доставчици на социални и здравни услуги. Ще бъдат осъществени следните основни дейности:1. Осигуряване на условия за предоставяне на услугата "Патронажна грижа"2. Регламентиране и обезпечаване реалното предоставяне на услугата "Патронажна грижа"3. Предоставяне на социално-здравната услуга „Патронажна грижа“.Изпълнението на проекта ще продължи 18 месеца, като се стартира с дейности, свързани с осигуряване условия за предоставяне на услугата, вкл. обучения на персонала, регламентиране на услугата за предоставяне в община Габрово, избор на външни изпълнители и др.Предоставяне на социално-здравната услуга „Патронажна грижа“ ще се осъществя за период от 12 месеца и ще отговори на нуждите на 224 потребителя.
