    Pour l'ensemble du projet d'insertion sociale et professionnelle, deux postes "d'accompagnatrices socio-professionnelles coordinatrices de parcours" ont été créés en 2017. Elles sont chargées du suivi individuel des bénéficiaires tout au long de leur parcours d'insertion et jusqu'à 6 mois après leur sortie d'un dispositif d'insertion sociale ou professionnelle du PISP afin de coordonner les étapes vers l'emploi. Leur mission sont : \- d'identifier, lors des instances de repérage mensuelles (hors périodes de vacances) qui réunissent l'ensemble des prescripteurs du territoire, quels seraient les dispositifs ou parcours les mieux à même de répondre à l'ensemble des enjeux posés concernant l'insertion des publics présentés. \- d'assurer un lien régulier (trimestriel) avec les bénéficiaires du PISP dans le suivi et la coordination des parcours d'insertion. Cette coordination est réalisée en lien avec le référent unique du Pôle Emploi (parcours à orientation professionnelle) ou du Conseil Départemental (parcours à orientation sociale) à l'origine des prescriptions vers les dispositifs d'insertion. Ce suivi valorise et renforce la cohérence des parcours personnels et questionne les acquis sur les différents dispositifs (AIS, ACI, AI, ETTI) en concertation avec les CIP, ASP ou animateurs chargés de l'accompagnement "dispositif". \- d'étayer et soutenir les bénéficiaires du PISP jusqu'à 6 mois après leur sortie d'un dispositif d'insertion afin, selon les situations de : pallier à des difficultés ponctuelles risquant de fragiliser l'insertion (adaptation au poste, relation à l'employeur, problématique mobilité ou matérielle,...), et poursuivre l'effort d'insertion déjà mis en place (suivi demande RQTH et lien avec les entreprises adaptées par exemple) \- de faciliter l'accès aux structures oeuvrant dans le champ de l'inclusion notamment dans les domaines de l'alphabétisation, de la mobilité, de la santé, du handicap, afin de rendre ces ensembles d'actions efficients pour l'insertion.
