Nous proposons une action de référence qui repose sur un accompagnement individualisé soutenu, prenant en compte la personne dans sa globalité, avec ses difficultés spécifiques et ses atouts. Il s'agit de l'aider à prendre conscience de ses difficultés comme de ces capacités, à se valoriser en élaborant ensemble - dans un cheminement progressif - les étapes nécessaires et pertinentes dans son parcours de retour à l'emploi et de reconquête d'une pleine autonomie socioprofessionnelle. Appuyée par un référent unique et interlocuteur privilégié, la personne est l'acteur principal de son parcours d'insertion. A ce titre, son adhésion et son engagement constant dans la démarche d'accompagnement sont fondamentaux. **_OBJECTIF(S) POURSUIVI(S) :_** * Faciliter l'insertion et l'accès à l'emploi des personnes les plus en difficulté et contribuer ainsi à la lutte contre les exclusions et à la réduction du chômage, * Favoriser l'accès à l'information pour leur permettre d'acquérir davantage d'autonomie, de lutter contre la discrmination, d'avoirs accès à leurs droits et à la culture, * Créer et/ou renforcer le lien social, * Permettre à ces personnes de retrouver une dynamique de projet, par l'instauration d'un lien interperosnnel soutenu et structurant et par des propositions de mises en activité, * Les aider à se revaloriser et à prendre conscience de leurs potentialités autant que de leurs difficultés spécifiques. We propose a reference action based on sustained individualised support, taking into account the person as a whole, with their specific difficulties and strengths. It is a question of helping him to become aware of his difficulties as well as of these abilities, to develop together — in a progressive way — the necessary and relevant steps in his path of returning to employment and regaining full socio-professional autonomy. Supported by a unique referent and privileged interlocutor, the person is the main actor in his or her integration path. As such, its adherence and constant commitment to the accompanying approach are fundamental. **_OBJECTIVE(s) FORSUIVI(S):_** * Facilitate the integration and access to employment of those most in difficulty and thus contribute to the fight against exclusions and the reduction of unemployment, * To promote access to information to enable them to gain greater autonomy, to combat discrimination, to have access to their rights and to culture, * Create and/or strengthen the social link, * To enable these people to regain a potential dynamic, through the establishment of a supportive and structuring interpersonal link and by means of proposals for their activities.
