**Mobility platform (**** Conseil en Mobilité Cœur d’Hérault/Montpellier et Cité Maritime** **Relay Mobility (Montpellier/Cœur d’Hérault/Béziers)** The overall project consists of 2 actions which have been in place for several years, apart from the posts of mobility adviser in the Maritime Cités and Montpellier territories: 1.Advisory on mobility: Individual reception of persons referred by prescribers to the mobility advisor. Information on existing transport and mobility assistance. Diagnosis on mobility needs and skills, support/guidance. Collective workshops on the theme of mobility. Guidance on any service “transport”, “mobility” according to the needs of the person 2\. Reception of the person guided by the mobility adviser (excluding the mobility adviser’s territories: Miscellaneous prescribers). Provision of a vehicle according to the need of the person. Regular monitoring visits, monitoring of the employment situation and follow-up on mobility. Return of the vehicle/reorientation to the mobility adviser. **Plateforme Mobilité (** **Conseil en Mobilité Cœur d’Hérault / Montpellier et Cité Maritime** **Relais Mobilité (Montpellier/ Cœur d'Hérault/ Béziers)** Le projet global se compose de 2 actions qui sont en place depuis plusieurs années, mis à part les postes de conseiller mobilité sur les territoires Cités maritimes et Montpellier : 1.Conseil en mobilité : Accueil individuel des personnes orientées par des prescripteurs vers le conseiller mobilité. Information sur l’existant en matière de transport et d’aide à la mobilité. Diagnostic sur les besoins et les compétences en mobilité, accompagnement / orientation. Ateliers collectifs autour du thème de la mobilité. Orientation sur toute prestation « transport », « mobilité » en fonction des besoins de la personne 2\. Relais mobilité Accueil de la personne orientée par le conseiller mobilité (hors territoires du conseiller mobilité : prescripteurs divers). Mise à disposition d’un véhicule en fonction du besoin de la personne. Visites de contrôle régulières, suivi de la situation au regard de l'emploi et des suites quant à la mobilité. Restitution du véhicule / réorientation vers le conseiller mobilité. Chaque personne est accompagnée tout au long des différentes phases pour la consolidation de la mobilité individuelle. Différentes actions peuvent être menées en parallèle. (French)
