The aim is to activate the spot. 60 on the age of 18-64 years in 3-20 for the period 13-02.2021 for the period 11.2017-. Action to be taken within the framework of the activity of the activity of active and social activation, already existing since 02.15 in the WMSON. Support based on an innovative model developed and implemented by the Eudojonia Foundation (HC OP). A model based on the concept of activation, tailored to the needs and possibilities of the ON, centred around the activation of the activity of the active coach (TA), with the possibility to benefit from the allowance. The first step is to carry out a thorough, multi-profile social diagnosis and fall. It aims to analyse the strengths of the project and diagnose the level of functioning of a project participant in the areas of social, occupational, family, education and health. The second phase shall be the determination of the competent diagnosis and the wording of the Individual Development Plan being drawn up jointly by the TA and the subject, and take action to improve the functioning of the environment. Activities carried out by TA with the additional support and intervention support of the super-visa, psychologist and full life adviser. As a complement to these activities, depending on the identified needs, other services offered, such as support for the super-visa, training courses, internships, activation packages or assistants’ services (including assistant ON, sign-language interpreter). The last step is to empower the participant, where TA has been withdrawn as the participant’s independence is supported. Realised support for dogs., cf. full life, supervision, training. and training courses in education., activities in support of employers, presentation of good practices. As a result, a minimum of 25 % of the BO will be employed, 34 % of the BO increases in the level of social security. The project is a development of CA in response to the demand of ON. (English)

    Celem jest aktywizacja społ-zaw. 60 ON w wieku 18-64 lat, zam.Olsztyn, pow.olsztyński,ostródzki i szczycieński w 3 edycjach (20 os x min. 13 m-cy)w okresie 11.2017-02.2021r. Działania realizowane w ramach istniejącego już od 02.15 r. w W-MSON Centrum Aktywności będącego podmiotem działającym na rzecz aktywizacji społ-zaw. Wsparcie oparte na innowacyjnym modelu Trener/-ka Aktywności opracowanym i wdrożonym przez Fundację Eudajmonia(POKL). Model oparty na idei aktywizacji, dostosowany do potrzeb i możliwości ON, skoncentrowany wokół aktyw. społ.-zaw. świadczonej przez trenera aktywności(TA) wraz z możliwością skorzystania z dodat. wsparcia. Pierwszym etapem jest przeprowadzenie dogłębnej, wieloprofilowej diagnozy społecz. i zawod. Ma na celu zanalizowanie mocnych stron oraz zdiagnozowanie poziomu funkcjonowania uczestnika projektu w obszarze życia społecznego, zawodowego, sytuacji rodzinnej, edukacyjnej i zdrowotnej. Drugim etapem jest przeprowadz. diagnozy właściwej i sformułowanie Indywidualnego Planu Rozwoju-wspólnie opracowywany przez TA oraz uczestnika, a dalej podjęcie działań na rzecz poprawy funkcjonowania w otoczeniu. Działania prowadzone przez TA przy dodatkowym i interwencyjnym wsparciu superwizora, psychologa i doradcy całożyciowego. Jako uzupełnienie tych działań, w zależności od zdiagnozowanych potrzeb oferowane inne usługi jak wsparcie superwizora, szkolenia zaw, staże,pakiety aktywizacyjne czy usługi asystenckie (w tym asystent ON, tłumacz jęz. migowego). Ostatnim etapem jest usamodzielnienie się uczestnika, gdzie TA w miarę usamodzielniania się uczestnika wycofuje się z wspierania.Realizowane wsparcie psych., por. całożyciowe,superwizja, szkol. i staże zaw. działania eduk., zajęcia gr.W celu wsparcia zatrud. prowadzone działania wspierające pracodawców-doradztwo, prezentowanie dobrych praktyk. Efektem proj.będzie zatrudnienie min. 25% BO,34% BO wzrost poziomu akt. społ. Projekt jest rozwinięciem działalności CA w odpowiedzi na zapotrzebowanie ON. (Polish)
