| Property / summary: The aim of the project is to increase social services, in particular community-based services and family support services and foster care through measures aimed at the equalisation of care deficits for families, with a view to reducing the placement of children in foster care and by action to move away from institutional to community-based services — the creation of two 14-personal care facilities. The activities of the project will be covered by 118 people. Children will receive comprehensive support, strengthening their social and educational competences. Parents will receive comprehensive social security assistance, including measures to strengthen parental roles and competences. The applicant undertakes to implement preventive actions to reduce the placement of children in a foster care (k.szczeg.8) through family support services and the activities of family assistants in a comprehensive manner. Family support and foster care take place in accordance with the law of 9.06.2011 on support for the family and foster care system and the implementing provisions of the Act and the “Pan-European guidelines for the transition from institutional to community based care” (kr.no.3) and “Use of EU funds for the transition from institutional to community-based care” — Toolbox. Celem projektu jest wzrost usług społ, w szczególności usług środowiskowych oraz usług wsparcia rodziny i pieczy zastępczej poprzez działania ukierunkowane na wyrównanie deficytów opiekuńczo-wychowawczych rodziny, mające ograniczyć umieszczanie dzieci w pieczy zastępczej oraz poprzez działania prowadzące do odejścia od opieki instytucjonalnej do usług świadczonych w lokalnej społeczności - stworzenie dwóch 14- osobowych placówek opiekuńczo-wychowawczych. Działaniami w projekcie zostanie objętych 118 osób. Dzieci otrzymają kompleksowe wsparcie, wzmacniające ich kompetencje społeczne i edukacyjne. Rodzice otrzymają kompleksową pomoc w zakresie funkcjonowania społecznego, obejmującą działania wzmacniające role rodzicielskie i kompetencje opiek-wychowawcze. Wnioskodawca zobowiązuje się do wdrożenia działań prewencyjnych ograniczających umieszczanie dzieci w pieczy zastępczej (kr.szczeg.8) poprzez usługi wsparcia rodziny i kompleksowe działania asystenta rodziny. Wsparcie rodziny i pieczy zastępczej odbywa się zgodnie z ustawą z 9.06.2011 o wspieraniu rodziny i systemie pieczy zastępczej i przepisów wykonawczych do ustawy oraz z "Ogólnoeuropejskimi wytycznymi dot. przejścia od opieki instytucjonalnej do opieki świadczonej na poziomie lokalnych społeczności" (kr.dost.3) i dokumentem "Wykorzystanie funduszy UE w celu przejścia od opieki instytucjonalnej do opieki świadczonej na poziomie lok. społeczności" - zestaw narzędzi.
