Projekt stanowi odpowiedź na problem zdrowotny występujący na terenie woj. dolnośląskiego, którym jest cukrzyca (typu 2). Projekt ma na celu zapobieganie i wczesne wykrywanie cukrzycy typu 2 wśród mieszkańców woj. dolnośląskiego. CELEM GŁÓWNYM PROJEKTU jest zmniejszenie liczby zachorowań na cukrzycę typu 2 wśród 6.020 mieszkańców (3.311K, 2.709M) subregionu wrocławskiego i m. Wrocław poprzez ich udział w działanniach edukacyjnych, badaniach profilaktycznych i konsultacjach w kierunku wykrywania cukrzycy typu 2 – cel zostanie osiągnięty do 30 czerwca 2023r. Planowane zadania są zgodne z założeniami Regionalnego Programu Zdrowotnego zapobiegania i wczesnego wykrywania cukrzycy typu 2 wśród mieszkańców woj. dolnośląskiego i obejmują: Etap 1 – działania informacyjno-edukacyjne, Etap 2 – działania diagnostyczne i Etap 3 – spotkania konsultacyjne (Etap 4 – ocena skuteczności i realizacji celów Programu, prowadzony będzie przez wnioskodawcę za pomocą ankiet zebranych od uczestników projektu oraz mierników efektywności realizacji programu. Dane będą gromadzone przez cały okres trwania projektu przez wnioskodawcę). Projekt skierowany jest do minimum 6.020 mieszkańców woj. dolnośląskiego (subregionu wrocławskiego i m. Wrocław), którzy są w wieku 35-64 lata i nie posiadają w dotychczasowym wywiadzie rozpoznanej cukrzycy typu 2 (dodatkowe kryterium wobec kobiet: kobiety nie będące w ciąży). Zakładanym wskaźnikiem rezultatu jest: Liczba osób, które dzięki interwencji EFS zgłosiły się na badanie profilaktyczne [6.020 osób]. W ramach projektu przewidziano możliwość realizacji wsparcia również w godzinach popołudniowych i wieczornych oraz w soboty. (Polish)

The project is a response to a health problem occurring in the Lower Silesian Voivodeship, which is diabetes (type 2). The project aims at preventing and early detection of type 2 diabetes among residents of the Lower Silesian Voivodeship. The main objective of the project is to reduce the number of cases of type 2 diabetes among 6.020 inhabitants (3.311K, 2.709M) of the Wrocław subregion and Wrocław through their participation in educational activities, prophylactic studies and consultations on the detection of type 2 diabetes — the target will be achieved by 30 June 2023. The planned tasks are consistent with the Regional Health Programme for Preventing and Early Detection of Type 2 Diabetes in Dolnośląskie Province and include: Stage 1 — Information and education activities, Stage 2 — diagnostic activities and Stage 3 — consultation meetings (Stage 4 — evaluation of the effectiveness and achievement of the objectives of the Programme, will be conducted by the applicant through surveys collected from project participants and measures of effectiveness of the programme implementation. The data will be collected throughout the duration of the project by the applicant). The project is aimed at a minimum of 6.020 inhabitants of the Lower Silesian Voivodeship (subregion of Wrocław and Wrocław), who are aged 35-64 and do not have diagnosed type 2 diabetes in the previous interview (an additional criterion for women: women who are not pregnant). The assumed result indicator is: Number of people who, thanks to ESF intervention, applied for a preventive study [6.020]. The project also provides for the possibility of providing support during the afternoon and evening hours and on Saturdays. (English)
