The project assumes the implementation of activities in the field of de-institutionalised forms of medical care for the elderly, including with disabilities, and actions affecting the improvement of the health of these people from the area of Kędzierzyn-Koziel County. The project will receive support from 32 people (including 40 % after hospitalisation in SP ZOZ Kędzierzyn-Koźle) and their 32 actual guardians. Four groups of patients (eight people each) will be supported as part of 12 weeks of care. Two weekly, 24-hour stays of the patient in a specialist centre (a week at the beginning and a week at the end of the patient’s support period) are assumed and the provision of 10 weeks of care for the patient at home. During the stay in the centre, the patient will undergo a specialist evaluation as part of the Global Geriatric Assessment conducted by a specialist in the field of geriatrics and a number of tests; specialist evaluation and diagnosis of cognitive disorders and mood disorders; assessment and diagnosis carried out by a nurse, a physiotherapist, a physiotherapist, a nutritionist and a speech therapist. Each assessment, depending on the needs, will end with making a diagnosis of a specialist and issuing recommendations, establishing an individual rehabilitation and therapy plan, implementing pharmacotherapy of cognitive function disorders and behavioural disorders with a preliminary evaluation of the effectiveness of treatment conducted by an experienced geriatric specialist. During the stay in the Senior Pension will be conducted: rehabilitation, occupational therapy, care of a medical caregiver and a qualified nurse. During the 10-week care after leaving the centre, telecare will be guaranteed 24 hours a day, telephone counselling and home visits of a doctor, nurse, dietitian, psychologist, rehabilitation and caregiver. In addition, the training of actual guardians (32 persons) is planned. (English)

    W ramach projektu zakłada się realizację działań z zakresu zdeinstytucjonalizowanych form opieki medycznej nad osobami starszymi, w tym z niepełnosprawnościami oraz działania mające wpływ na poprawę stanu zdrowia tychże osób z terenu powiatu kędzierzyńsko-kozielskiego. W przedsięwzięciu wsparcie otrzymają 32 osoby (w tym 40% po zakończonej hospitalizacji w SP ZOZ Kędzierzyn-Koźle) oraz ich 32 opiekunów faktycznych. Wsparciem objęte zostaną 4 grupy pacjentów (po 8 osób każda), w ramach 12-sto tygodniowej opieki. Zakłada się dwa tygodniowe, całodobowe pobyty pacjenta w specjalistycznym ośrodku (tydzień na początku jak i tydzień na koniec okresu wsparcia pacjenta) oraz zapewnienie 10-cio tygodniowej opieki nad pacjentem w warunkach domowych. Podczas pobytu w ośrodku pacjent poddany zostanie specjalistycznej ocenie w ramach Całościowej Oceny Geriatrycznej przeprowadzonej przez lekarza specjalistę z dziedziny geriatrii oraz szeregowi badań; specjalistycznej ocenie i diagnozie zaburzeń funkcji poznawczych oraz zaburzeń nastroju; ocenie i diagnostyce przeprowadzonej przez pielęgniarkę, rehabilitanta, fizjoterapeutę, dietetyka oraz logopedę. Każda ocena, w zależności od potrzeb, zakończy się postawieniem diagnozy specjalisty oraz wydaniem zaleceń, ustaleniem indywidualnego planu rehabilitacji oraz terapii, wdrożeniem farmakoterapii zaburzeń funkcji poznawczych oraz zaburzeń zachowania ze wstępną oceną skuteczności leczenia prowadzoną przez doświadczonego specjalistę geriatrę. Podczas pobytu w Pensjonacie Seniora prowadzona będzie: rehabilitacja, terapia zajęciowa, opieka opiekunki medycznej oraz wykwalifikowanej pielęgniarki. Podczas 10-cio tygodniowej opieki po opuszczeniu ośrodka zagwarantowana zostanie każdemu pacjentowi teleopieka 24h/dobę, porady telefoniczne oraz wizyty domowe lekarza, pielęgniarki, dietetyka, psychologa, rehabilitanta oraz opiekunki. Dodatkowo zaplanowano szkolenia opiekunów faktycznych (32os.). (Polish)
