One of the most pressing issues visible in the Druskininkai and Grodno regions in recent decades is population decline. Low birth rate and negative net migration, premature mortality, lead to population decline, also increase the proportion of older people within the total population. Increasing morbidity rates, low participation in prevention programs, especially of inhabitants living in the rural areas, leads to poor health. Aging population increases demand of special healthcare services, such as nursing services. Accessibility and quality of health care services, provided in the region are insufficient: health and other professionals lack knowledge how effectively deal with such issues, insufficient infrastructure does not create opportunities for effective healthcare provision and management, as well as access, the awareness and health literacy of target groups also is insufficient. The main goal of the project is to increase quality of life of elderly people and disabled in Druskininkai and Grodno regions by increasing accessibility and improving the quality of existing primary out-patient and nursing health services in the regions. During Project implementation by coordinating integrated actions positive results are expected. The health literacy and awareness of mentioned vulnerable groups will be increased, health professionals will share and improve their knowledge and the necessary health infrastructure will be developed in order to ensure equal access (to a maximum possible extent) to a high-quality public healthcare services. Interventions envisaged in the Project will help to improve services relevant for indicated vulnerable groups, to create preconditions for longer and healthier living for these population groups, lower risks of premature deaths, social exclusion and poverty.
