Projekto tikslas - sukurti ir plėtoti kokybišką integralią pagalbą (socialinės globos ir slaugos) namuose neįgaliems vaikams, darbingo amžiaus neįgaliems asmenims, senyvo amžiaus asmenims ir konsultacinę pagalbą šeimos nariams, prižiūrintiems savo artimuosius. Projektą "Integralios pagalbos (socialinės globos ir slaugos) namuose paslaugų plėtra Trakų rajono savivaldybėje" vykdys Trakų globos ir socialinių paslaugų centras. Bendradarbiaujant su sveikatos priežiūros įstaigomis ir Vokietijos Reinės Carito draugija bus tobulinamas 2013-2015 metais suformuotas integralios pagalbos modelis, tikslingai nukreipiamos paslaugos ir resursai. Integralios pagalbos paslaugas kiekvieną mėnesį gaus virš 20 asmenų, projekto vykdymo metu gavėjų skaičius didės iki 95, nes mirusius ar dėl kitų priežasčių iš projekto išėjusius projekto dalyvius pakeis nauji. Šeimos nariams, prižiūrintiems savo artimuosius, bus teikiama konsultacinė pagalba (ją gaus 30 šeimos narių). Atsižvelgiant į sukauptą darbo patirtį, bus suformuotos 3 mobilios komandos iš skirtingų specialistų. Vienoje komandoje dirbs slaugytojai ir jų padėjėjai, socialiniai darbuotojai ir jų padėjėjai ir aptarnaus 7-15 paslaugų gavėjų, iš viso numatyta apie 30,5 etato. Siekiant kokybiškų paslaugų, komandos nariai dalyvaus kompetencijos ir gebėjimų stiprinimo mokymuose. Projekte dalyvaus 15 savanorių, kurie taip pat bus apmokyti. Siekiant skatinti savanorišką veiklą ir ją viešinti, planuojami 6 renginiai savanoriams ir bendruomenių nariams, kurie bus organizuojami seniūnijose, arčiau bendruomenių. Projektas bus vykdomas iš ES struktūrinių fondų paramos lėšų ir užtikrins integralios pagalbos namuose tęstinumą, jų prieinamumą ir plėtotę, bus teikiamos paslaugos neįgaliems ir senyvo amžiaus asmenims ir juos prižiūrintiems šeimos nariams. Projektas įgalins koncentruoti kompleksinės pagalbos teikimą vienoje įstaigoje ir sudarys didesnes galimybes asmenims, negalintiems pasirūpinti savimi, būti lygiaverčiais visuomenės nariais (Lithuanian)

The aim of the project is to create and develop quality integral assistance (social care and care) at home for disabled children, disabled people of working age, elderly people and advisory assistance to family members caring for their relatives. The project “Development of Integrated Assistance (Social Care and Care) in Homes in Trakai District Municipality” will be carried out by the Trakai Care and Social Services Centre. In cooperation with the health care institutions and the German Society of Reine Carito, the Integral Assistance Model developed in 2013-2015 will be improved and the services and resources targeted. More than 20 people will receive integrated support services each month, and the number of beneficiaries will increase to 95 during the project, as the participants of the project will be replaced by new participants who have died or otherwise left the project. Family members caring for their relatives will receive advisory assistance (30 family members). Based on the experience gained, 3 mobile teams from different specialists will be formed. One team will include nurses and their assistants, social workers and their assistants and will serve between 7 and 15 service recipients, with a total of approximately 30.5 posts. For quality services, team members will participate in competence and capacity building training. The project will involve 15 volunteers who will also be trained. In order to promote and publicise volunteering, 6 events are planned for volunteers and community members to be organised in the elderships, closer to communities. The project will be implemented with support from the EU Structural Funds and will ensure continuity, accessibility and development of Integral Home Support, and will provide services for disabled and elderly people and family members caring for them. The project will enable the concentration of cross-cutting assistance in one institution and increase opportunities for those who are unable to take care of themselves to be equal members of society (English)
