The project aims at improving the quality of life and opportunities for social inclusion of people with disabilities and older people over 65 years old, incapable of self-service on the territory of Burgas Municipality, by providing a network of services in the home environment and building appropriate capacity for their provision. The project foresees the provision of integrated health and social services in the homes of 241 deprived persons with disabilities and elderly people over 65 years old, incapable of self-service, according to their individual needs. To ensure adequate conditions for the effective provision of patronage care, planned activities include carrying out repairs and fitting out premises, purchasing a means of transport for the needs of the patronage care team. The project will respond to the need to provide adequate support and creation of conditions for persons with disabilities and older people over 65 years old, in the inability of self-service to receive quality and accessible services based on their individual needs. Implementation of the project will contribute to providing quality complex care for people with disabilities and older people over 65 years old, incapable of self-service through the provision of health care and social services. Проектът е насочен към подобряване качеството на живот и възможностите за социално включване на хората с увреждания и възрастните хора над 65г. в невъзможност от самообслужване на територията на Община Бургас, чрез осигуряване на мрежа от услуги в домашна среда и изграждане на подходящ капацитет за предоставянето им. В рамките на проекта се предвижда предоставяне на интегрирани здравно-социални услуги в домовете на 241 нуждаещи се лица с увреждания и възрастни хора над 65г. в невъзможност от самообслужване, съгласно индивидуалните им потребности. За осигуряване на подходящи условия за ефективно предоставяне на патронажната грижа, планираните дейности включват извършване на ремонт и оборудване на помещения, закупуване на транспортно средство за нуждите на екипа осигуряващ патронажните грижи. Проектът ще отговори на необходимостта от оказване на адекватна подкрепа и създаване на условия за лица с увреждания и възрастните хора над 65г. в невъзможност от самообслужване да получат качествени и достъпни услуги, основани на индивидуалните им нужди.Реализиране на проекта ще допринесе за осигуряване на качествена комплексна грижа за хората с увреждания и възрастните хора над 65г. в невъзможност от самообслужване чрез предоставяне на здравни грижи и социални услуги.
