The project proposal is intended to provide integrated cross-sectoral services for two target groups: people with disabilities and lonely living people over 65 with inability to self-service. Following a targeted and comprehensive needs assessment, in relation to individual service plans, project activities shall be targeted at the most deprived persons, depending on the risk and identified needs. The project activities will involve more than 60 people who will receive planned care and support to improve their living status, overcome the risk of poverty, achieve better physical and mental health, better social inclusion. The activities relate to the provision of supporting services, including integrated cross-sectoral community or home environments; improving access to healthcare; activities in the community aimed at full social inclusion with a cultural, educational and professional nature. All activities and services are planned according to the needs of the individual and the needs of the individual are placed at the centre of their implementation. The services shall be combined in accordance with the individual needs of the individual, promoting an integrated approach and ensuring that the needs of the individual are comprehensively met, according to their individual assessment. The services are provided by a multidisciplinary team — servicers (social workers), specialists, experts and consultants with different educational and professional background profiles. In parallel with the support provided to tackle everyday life, access to healthcare, social inclusion services, the project is also working to change societal negative attitudes related to existing hidden discrimination and stigma towards people with disabilities and elderly single people. Проектното предложение е предназначено за предоставяне на интегрирани междусекторни услуги за две целеви групи: хора с увреждания и самотно живеещи хора над 65г. с невъзможност за самообслужване. След целенасочена и обстойна оценка на потребностите, във връзка с изготвени индивидуални планове за предоставяне на услуги, проектните дейности се насочват към най-нуждаещите се лица, в зависимост от риска и установените нужди. В дейностите на проекта ще бъдат обхванати над 60 човека, които ще получат планирани грижа и подкрепа с цел подобряване на техния жизнен статус, преодоляване на риска от бедност, постигане на по-добро физическо и психично здраве, по-пълноценно социално включване. Дейностите са свързани с предоставянето на подкрепящи услуги в т.ч. интегрирани междусекторни в общността или домашна среда; подобряване достъпа до здравеопазване; дейности в общността насочени към пълноценно социално включване с културен, обучителен и с професионална насоченост характер. Всички дейности и услуги се планират според потребностите на лицето като нуждите на отделния човек се поставят в центъра на тяхното изпълнение. Услугите се комбинират в съответствие с индивидуалните потребности на лицата като се насърчава интегрирания подход и се осигурява комплексно удовлетворяване на потребностите на отделната личност, според индивидуалната му оценка. Услугите се предоставят от мултидисциплинарен екип - обслужващи лица (сътрудници социални дейности), специалисти, експерти и консултанти с различен профил на завършено образование и професионален стаж. Успоредно с оказваната подкрепа за справяне в ежедневието, достъпа до здравеопазване, услугите за социално включване, по проекта се работи и за промяна на обществените негативни нагласи, свързани със съществуваща скрита дискриминация и стигма по отношение на хората с увреждания и възрастните самотни хора.
