The aim of the project proposal is to overcome social exclusion and poverty by improving the quality of life and access to social and health services for people with disabilities and people over 65 years old, unable to self-service, in response to their complex needs. With the project Byala Municipality creates a new Center for hourly provision of services for social inclusion in the community or in a home environment, including supporting and integrated social and health services at municipal level. The services will be provided on the basis of an individual social assessment and the specific needs of those in need. The project will also support the families of the targeted people by enabling them to return to the labour market. The Centre will employ the following staff necessary for its overall functioning and provision of services: manager-1, social worker-1, social assistant-5, home assistant/healthcare — 81, and staff for specialised services nurse-1, psychologist-1, pedagogue -1 and rehabilitator -1. The staff for the provision of services will be trained and will be provided psychological support and/or counselling. Service users will be motivated and/or psychologically supported according to their individual needs. Specialised transport for people with disabilities will be hired to transport the target group to provide specialised services in the Centre and in the community. Furniture and equipment will be purchased for the functioning of the Centre. A project management team will be set up, consisting of: manager, accountant and technical assistant. Publicity and dissemination of information on the specific objectives and results of the project and the EU financial contribution will be provided. The project will contribute to promoting equal opportunities for vulnerable groups and expanding the volume and diversity of social services provided in the community or in a home environment in the municipality of Byala.     Целта на проектното предложение е преодоляване на социалното изключване и бедността, чрез подобряване качеството на живот и достъпа до социални и здравни услуги на хора с увреждания и хора над 65 г. в невъзможност за самообслужване, в отговор на комплексните им потребности. С проекта Община Бяла създава нов Център за почасово предоставяне на услуги за социално включване в общността или в домашна среда, в т.ч. подкрепящи и интегрирани социални и здравни услуги на общинско ниво. Услугите ще се предоставят въз основа на индивидуална социална оценка и специфичните потребности на нуждаещите се лица. С проекта ще се подкрепят и семействата, на хората от целевата група, като им се даде възможност за връщане на пазара на труда. В Центъра ще бъде нает следния персонал, необходим за цялостното му функциониране и предоставяне на услугите: управител-1 бр., социален работник-1 бр., социален асистент-5 бр., домашен помощник/санитар-10 бр. , личен асистент -81 бр. и персонал за специализирани услуги-медицинска сестра-1 бр., психолог-1 бр., педагог -1 бр. и рехабилитатор -1 бр. Персоналът за предоставяне на услуги ще бъде обучен и ще му се предостави психологическа подкрепа и/или консултиране. На потребителите на услуги ще се окаже мотивационна и/или психологическа подкрепа, според индивидуалните им потребности. Ще се наеме специализиран транспорт за хора с увреждания за транспортиране на лицата от целевата група за предоставяне на специализирани услуги в Центъра и в общността. За функционирането на Центъра ще бъдат закупени обзавеждане и оборудване. Ще бъде сформиран екип за управление на проекта, в състав: ръководител, счетоводител и технически сътрудник. Ще бъде осигурена публичност и разпространена информация относно конкретните цели и резултати от проекта и финансовия принос на ЕС. Проектът ще допринесе за насърчаване равните възможности на уязвимите групи и разширяване обема и многообразието на социалните услуги, предоставяни в общността или в домашна среда в община Бяла.
