Depuis sa création, ARPS Synergence propose aux personnes présentant des difficultés psychiques, des actions en vue de favoriser leur insertion socio- professionnelle et ceci en complémentarité des dispositifs existants sur le département de l'Indre et Loire. Une équipe pluridisciplinaire et mobile, et au coeur d'un réseau de compétences des champs sanitaires, médico-social et de l'insertion du Département d'Indre et Loire propose cinq prestations : analyse partagée de la situation (accueil), bilan d'appui à l'insertion, ré- entrainement/accompagnement, maintien et appui à l'intégration et une fonction permanente de ressource et conseils. _**La prestation 1 d'accueil**_ vise à prendre contact avec la personne, à créer un lien de confiance, à l'aider à se positionner au regard de son parcours sur une éventuelle entrée en bilan d'appui à l'insertion et à éclairer le partenaire à l'origine de la mise en relation sur la problématique de la personne. _**Le bilan d'appui à l'insertion**_ a ensuite pour objectif d'être une étape de réflexions partagées avec la personne autour de ses freins et de ses potentiels pour son parcours d'insertion, mais aussi autour des hypothèses d'insertion et des accompagnements complémentaires fréquemment nécessaires. Cette étape est le plus souvent inscrite dans une dynamique de changement pour la personne. A l'issue du bilan, les personnes peuvent intégrer la _**prestation 3 de ré- entrainement/ accompagnement**_ dont l'objectif est d'aider à l'acheminement d'objectifs individuels permettant une avancée vers l'insertion socio- professionnelle. Ses actions s'inscrivent dans une logique de poursuite et de cohérence avec l'étape du bilan d'appui à l'insertion proposée en amont. L'objectif est de maintenir les personnes dans une dynamique, d'éviter des ruptures dans leur parcours malgré une fragilité encore présente par la poursuite d'un travail de projection personnelle et professionnelle. La _**prestation 4 de ressource et conseil**_ concerne toutes les sollicitations qu'ARPS Synergence reçoit des professionnels, des familles, des personnes souffrant de difficultés psychiques directement. Celles-ci peuvent être physiques mais le plus souvent se font par téléphone et visent à apporter un éclairage sur la situation d'une personne avec des difficultés psychiques rencontrant une problématique d'insertion. La _**prestation 5 d'appui à l'intégration et au maintien**_ (en emploi, en entreprise ou en formation) vise à répondre aux sollicitations de personnes, d'organismes, d'entreprises par l'apport d'accompagnements spécifiques et pluridisciplinaires d'ARPS Synergence et de propositions de compensations. *-*-*-*-* En 2017, en lien avec la négociation du CPOM d'ARPS Fontenailles CPO-CRP, établissement géré par l'ARPS, l'ARS Centre-Val de Loire a souhaité engager avec l'ARPS une réflexion sur l'évolution d'ARPS Synergence, en lien avec l'Emploi Accompagné. Cette démarche d'Emploi Accompagné, faisant l'objet d'un Appel à Candidature ouvert par l'ARS et conduit par ARPS Fontenailles CPO-CRP, pour des bénéficiaires et des territoires ciblés, vise à un accompagnement médico- social individualisé des personnes en situation de handicap pour accéder et/ou se maintenir dans un emploi en milieu ordinaire de travail. Une proposition (jointe en annexe) a été adressée à l'ARS Centre-Val de Loire à l'été 2018, permettant l'évolution de l'accompagnement d'ARPS Synergence en l'inscrivant dans une démarche "Emploi Accompagné", et proposant une évaluation de la situation, la détermination d'un projet professionnel et l'accompagnement et/ou le maintien en milieu ordinaire de travail. **_A partir de septembre 2018_** , les salariés d'ARPS Synergence, **qui devient ARPS Synea** , ont entrepris de mettre en application cette évolution d'accompagnement, mais ont fait évoluer leurs pratiques, au regard du projet adressé à l'ARS Centre-Val de Loire, prenant en considération : * la réalité du territoire couvert, * l'intensité de l'accompagnement induit par les principes de fonctionnement de l'Emploi Accompagné, * l'expérience des référents Emploi Accompagné intervenant dans le cadre du dispositif Emploi Accompagné financé par l'Appel à Candidature de l'ARS Centre-Val de Loire, * et la volonté de faire d'ARPS Synea un dispositif intervenant en complétude de ce dispositif (tant sur le territoire couvert que sur les bénéficiaires). Il faut tout de même noter que l'accompagnement projeté dans le projet adressé à l'ARS Centre-Val de Loire, présentait les mêmes finalités que le dispositif Emploi Accompagné financé au titre de l'Appel à candidature, et proposait les mêmes modes d'accompagnement. ARPS Synea, répond à plusieurs caractéristiques : * accueillir des personnes présentant des troubles psychiques, neurologiques et/ou une déficience intellectuelle ; * compléter le dispositif Emploi Accompagné financé par l'ARS Centre Val de Loire dans le cadre de l'appel à candidature et conduit par ARPS F (French)

Since its inception, ARPS Synergence has been offering people with mental difficulties actions to promote their socio-professional integration, complementing the existing arrangements in the department of Indre et Loire. A multidisciplinary and mobile team, and at the heart of a network of competences of the health, medico-social and integration fields of the Department of Indre et Loire offers five services: shared analysis of the situation (reception), assessment of support for integration, retraining/accompanyment, maintenance and support for integration and a permanent function of resources and advice. _**The purpose of the reception service is to contact the person, to create a bond of trust, to help him/her to position himself in view of his or her career on a possible inclusion support record and to inform the partner responsible for the relationship on the person’s problem. _**The balance sheet of support for insertion**_ then aims to be a stage of reflections shared with the person around his or her brakes and potentials for his or her insertion path, but also around the insertion hypotheses and complementary accompaniments frequently needed. This step is most often embedded in a dynamic of change for the person. At the end of the assessment, people can take part in the 3 retraining/accompaniment benefit**_ the purpose of which is to assist in the delivery of individual objectives enabling progress towards socio-occupational integration. Its actions are consistent with and consistent with the stage of the upstream inclusion support review. The aim is to keep people in a dynamic, to avoid breaks in their paths despite a fragility still present by the pursuit of personal and professional projection work. The _**resource provision and advice**_ concerns all requests that ARPS Synergence receives from professionals, families, people suffering from psychic difficulties directly. These can be physical, but most often are done by telephone and aim to shed light on the situation of a person with mental difficulties encountering a problem of insertion. The _**service 5 support for integration and maintenance**_ (in employment, in enterprises or in training) is intended to respond to requests from individuals, organisations and enterprises by providing specific and multidisciplinary support from ARPS Synergence and proposals for compensation. *-*-*-*-* In 2017, in connection with the negotiation of the CPOM of ARPS Fontenailles CPO-CRP, an establishment managed by ARPS, the ARS Centre-Val de Loire wished to initiate a reflection with ARPS Synergence on the evolution of ARPS Synergence, in connection with Accompanied Employment. This Accompanied Employment approach, which is the subject of a call for applications opened by the ARS and led by ARPS Fontenailles CPO-CRP, for targeted beneficiaries and territories, aims to provide individualised medico-social support for people with disabilities in order to access and/or stay in a job in the ordinary working environment. A proposal (attached in annex) was sent to ARS Centre-Val de Loire in the summer of 2018, allowing the development of ARPS Synergence’s support by including it in a “Employment Accompanied” approach and proposing an assessment of the situation, the determination of a professional project and the support and/or maintenance in the ordinary working environment. **_As of September 2018_**, the employees of ARPS Synergence, **which becomes ARPS Synea**, have undertaken to implement this development of support, but have changed their practices, in view of the project addressed to ARS Centre-Val de Loire, taking into account: * the reality of the territory covered, * the intensity of the support induced by the operating principles of the Accompanied Employment, * the experience of the referents Emploi Accompagné operating within the framework of the Emploi Accompagné scheme financed by the Call for Candidature of the ARS Centre-Val de Loire, * and the desire to make ARPS Synea a device involved in the completion of this scheme (both in the area covered and on the beneficiaries). It should be noted, however, that the support planned in the project addressed to ARS Centre-Val de Loire, had the same aims as the Accompanied Employment scheme funded under the Call for Applications, and proposed the same methods of support. Arps Synea, has several characteristics: * to accommodate persons with mental, neurological and/or intellectual disabilities; * complete the Accompanied Employment scheme funded by the ARS Centre Val de Loire as part of the call for applications and led by ARPS F (English)
