Project (P) “Development of the care services system in the town of Ostrów Mazowiecka” includes measures to facilitate access to social services for 60 dependent persons from the town of Ostrów Mazowiecka (Mazowieckie province) over the 24-month period of the project, by increasing the scope and volume of social services conducive to deinstitutionalisation. The P will be implemented on the basis of the ‘Pan-European guidelines for the transition from institutional to community-based care’ and the provisions of the ‘Minimum requirements for the provision of social services’. It will also use the tools described in the document “Use of European Union funds for the transition from institutional to community-based care — toolbox”. Project participants (UP) will receive support in the form of care services and/or telecare, psychological counselling and rehabilitation. The use of an innovative method, i.e. tele-care, will ensure efficient forms of monitoring for dependent persons while respecting their privacy and the use of their resources to allow them to function in their environment while minimising the risk of threats to their health and life. The project will also set up a rehabilitation equipment for UP to meet immediate needs. In addition, in the framework of the project we plan to provide support to 30 carers of genuine carers. Through education on care of dependants, tutoring and psychological counselling will motivate people to effectively plan their time and take up working lives. (English) | |||

Projekt (P) pn. "Rozwój systemu usług opiekuńczych na terenie miasta Ostrów Mazowiecka"obejmuje działania na rzecz ułatwienia dostępu do usług społecznych dla 60 osób niesamodzielnych z terenu miasta Ostrów Mazowiecka (woj. mazowieckie) w ciągu 24 miesięcy trwania projektu poprzez zwiększenie zakresu i ilości usług społecznych sprzyjających deinstytucjonalizacji. P realizowany będzie w oparciu o "Ogólnoeuropejskie wytyczne dotyczące przejścia z opieki instytucjonalnej do opieki świadczonej na poziomie lokalnych społeczności" oraz z zapisami dokumentu "Minimalne wymagania świadczenia usług społecznych". Wykorzystane zostaną w nim również narzędzia opisane w dokumencie "Wykorzystanie funduszy Unii Europejskiej w celu przejścia od opieki instytucjonalnej do opieki świadczonej na poziomie lokalnych społeczności- zestaw narzędzi". W ramach projektu 60 uczestników projektu (UP) otrzyma wsparcie w postaci usług opiekuńczych i/lub teleopieki oraz poradnictwa psychologicznego i rehabilitacji. Zastosowanie innowacyjnej metody tj. teleopieki pozwoli na zapewnienie efektywnej formy monitorowania osób niesamodzielnych przy zachowaniu ich prywatności i wykorzystaniu posiadanych przez nich zasobów co pozwoli jednocześnie na ich funkcjonowanie w swoim środowisku przy zminimalizowaniu ryzyka wystąpienia zagrożenia dla ich zdrowia i życia. W ramach projektu utworzona zostanie również wypożyczalnia sprzętu rehabilitacyjnego, który służyć będzie UP do zaspokajania doraźnych potrzeb w funkcjonowaniu. Dodatkowo w ramach projektu planujemy objąć wsparciem 30 opiekunów faktycznych sprawujących opiekę nad osobami niesamodzielnymi. Poprzez szkolenia edukacyjne z zakresu opieki nad osobami niesamodzielnymi, zajęcia w ramach grup wsparcia oraz poradnictwo psychologiczne zostaną zmotywowani do efektywnego planowania swojego czasu oraz podejmowania aktywności zawodowej.
