Coordinated interdisciplinary project, in which the objectives of the Microbioma subproject are: The main ones. 1) Characterise and compare the diversity of the microbiome of the lower respiratory tract of patients with severe community pneumonia requiring mechanical ventilation with that of control patients without infectious respiratory disease. 2) To establish the relationship between the different levels of serum biomarkers and the respiratory microbiome in patients with severe pneumonia. Secondary school. 3) Compare the respiratory microbiome between patients with viral and bacterial pneumonia and in the course of infection. 4) Determine the etiology and diagnostic role of new NGS techniques in patients with severe pneumonia. 5) Analyse the value of molecular techniques in the diagnosis of severe pneumonia in the patient with previous antibiotic treatment. The respiratory microbiome of bronchoalveolar mini-washing will be studied in 50 patients with community-acquired pneumonia with mechanical ventilation and 20 control patients using the Ion Torrent platform and the ION 16S metagenomics KIT (Life Technologies) kit. The same patients will receive a comparatively comprehensive microbiological diagnosis with routine techniques: bacterial and fungal culture, virus detection by specific PCR, detection of pneumococcal antigens and Legionella in urine. This study will provide information on changes in the respiratory microbiome in severe community pneumonia, depending on its bacterial and viral origin, will improve the current diagnostic capacity of pneumonia and will confirm or design new empirical strategies to be included in antibiotic treatment clinical guidelines. Proyecto coordinado interdisciplinar, en el que los objetivos del subproyecto Microbioma son: Principales. 1) Caracterizar y comparar la diversidad del microbioma del tracto respiratorio inferior de pacientes con neumonía comunitaria grave que requieran ventilación mecánica con la de pacientes control sin patología respiratoria infecciosa. 2) Establecer la relación de los diferentes niveles de biomarcadores séricos con el microbioma respiratorio en pacientes con neumonía grave. Secundarios. 3) Comparar el microbioma respiratorio entre pacientes con neumonía viral y bacteriana y en el transcurso de la infección. 4) Determinar la etiología y el papel diagnóstico de las nuevas técnicas de NGS en pacientes con neumonía grave. 5) Analizar el valor de las técnicas moleculares en el diagnóstico de la neumonía grave en el paciente con tratamiento antibiótico previo. Para ello se estudiará el microbioma respiratorio del mini-lavado broncoalveolar de 50 pacientes con neumonía adquirida en la comunidad con ventilación mecánica y de 20 pacientes control utilizando la plataforma de Ion Torrent y el kit ION 16S METAGENOMICS KIT (Life Technologies). A los mismos pacientes se les realizará comparativamente un diagnostico microbiológico exhaustivo con técnicas rutinarias: cultivo bacteriano y de hongos, detección de virus mediante PCR específica, detección de antígenos de neumococo y Legionella en orina. Este estudio aportará información sobre las modificaciones del microbioma respiratorio en la neumonía comunitaria grave, en función de su origen bacteriano y viral, mejorará la capacidad diagnóstica actual de la neumonía y permitirá confirmar o diseñar nuevas estrategias empíricas a incluir en las guías clínicas de tratamiento antibiótico.
