Projekt se zaměřuje na nalezení a udržení stabilního bydlení u lidí se závažným duševním onemocněným ohrožených bezdomovectvím. Nejvíce je bezdomovectvím ohrožena ta část lidí z CS, která vypadává ze zdravotní a sociální péče. Tato část bude aktivně vyhledána a podpořena službou sociální rehabilitace. Vyhledání proběhne jak ve zdravotnických zařízeních, tak prostřednictvím spolupráce s majiteli bytů. Cílem je stabilizace bytové situace a zvýšení schopností pro zvládání samostatného života. (Czech)

The project focuses on finding and maintaining stable housing in people with severe mental illnesses at risk of homelessness. Homelessness is most endangered by the part of CS people who are falling out of health and social care. This section will be actively sought and supported by the social rehabilitation service. The search will take place both in medical facilities and through cooperation with the owners of the apartments. The aim is to stabilise the housing situation and increase the ability to manage independent life. (English)
