Projekt (typ 1) jest skierowany do 64 osób (44K) zagrożonych ubóstwem i wykluczeniem społecznym, niesamodzielnych, w tym niepełnosprawnych, zamieszkałych na terenie woj. zach-pom w Gminie Krzęcin oraz ich 10 opiekunów faktycznych (8K). Celem głównym proj jest zwiększenie liczby os objętych usług społ. w Gminie Krzęcin o 64 os niesamodzielnych, w tym 44 kobiet (dalej: K), mającego służyć zaspokojeniu potrzeb tych os wynik. z niesamodzielności poprzez m.in. usługi opiekuńcze świadczone w miejscu zamieszkania, specjalistyczne usługi opiekuńcze, Klub Seniora oraz zwiększenie wiedzy 10 opiekunów faktycznych (8K) z zakr. opieki nad podopiecznymi w okr. 01.01.2020- 31.10.2021. Proj odpowiada na problemy i potrzeby w świadczeniu usług społ., zidentyfikowane na obszarze jego real. biorąc pod uwagę trendy demograf. i poziom dostępności usług społ. na tym obszarze. Proj będzie skierowany do os niesamodzielnych, w tym niepełnos. Zadania w ramach proj: wsparcie opiekuńcze, opiekuńcze specjal., centrum wsparcia opiekunów faktycznych, Klub Seniora, co przyczyni się do zwiększenia autonomii UP, zwiększenie dostępności i bezpieczeństwa dla podopiecznych. Usługi będą świadczone w oparciu o wysokie standardy przez specjalistyczną kadrę, przygotowaną do świadczenia tych usług, w zdecydowanej większości w miejscach zamieszkania. Usługi te będą służyć zaspokojeniu rosnących potrzeb wynikających z niesam. Dzięki proj 64 osób niesam. (44K) będzie mogło otrzymać niezbędne wsparcie poza systemem instytucjonalizowanej pomocy, przez co dłużej będą mogły być aktywne w społeczności lokalnej. Rezultaty: 1. Liczba miejsc świadczenia usług społ. istniejących po zakończeniu projektu: 8 (2 opiekunki, 1 pielęgniarka, 1 rehabilitant, 1 psycholog, 3 lokalizacje Klubu Seniora) 2. The project (type 1) is aimed at 64 people (44K) at risk of poverty and social exclusion, dependent, including the disabled, living in the Krzęcin Commune and their 10 actual carers (8K). The main aim of the project is to increase the number of wasps covered by community services in Krzęcin Municipality by 64 dependants, including 44 women (hereafter: K), intended to meet the needs of these people’s outcome. from independence through, among others, care services provided at the place of residence, specialised care services, Senior Club and increased knowledge of 10 actual carers (8K) from the area of care of the patients in the area. 01.01.2020-31.10.2021. The project responds to problems and needs in the provision of social services, identified in its real area. taking into account trends demograph. and the level of availability of social services in this area. The project will be directed at the wasps, including the disability. Project tasks: care support, caring specialty, support center for actual caregivers, Senior Club, which will help increase the autonomy of UP, increase availability and security for patients. The services will be provided on the basis of high standards by specialist staff, prepared for the provision of these services, most of them at the place of residence. These services will serve to meet the growing needs of the unsustainable ones. Thanks to the project, I'm a total of 64 people. (44K) will be able to receive the necessary support outside the institutionalised assistance system, which will allow them to be active in the local community for longer. Results: 1. Number of places to provide community services after completion of the project: 8 (2 babysitters, 1 nurse, 1 rehabilitant, 1 psychologist, 3 locations of the Senior Club) 2.
