Celem głównym proj. jest zwiększenie dostępu do usług społecznych dla 60 os. (46K,14M) zagrożonych ubóstwem lub wykluczeniem społ., stanowiących grupę docelową proj., w tym 10 os. (8K,2M) z niepełnosprawnościami, zamieszkujących Gm. Nowa Sucha (woj. mazowieckie), poprzez utworzenie i zapewnienie bieżącego funkcjonowania Klubu Seniora (miejsca świadczeń usług opiekuńczych) w Nowej Suchej w okresie od 1.11.2018 do 31.10.2020. W proj. zaplan. realizację 2 zadań: 1. Utworzenie dziennej formy usług opiekuńczych w postaci Klubu Seniora W ramach zadania zostaną dostos. i doposażone pomieszczenia w budynku GOKSiR w Nowej Suchej, w którym będzie funkcj. nowo utworzony Klub Seniora. 2. Bieżące funkcjonowanie Klubu Seniora. Dla uczestników projektów zaplanowano: a) zajęcia mające na celu działalność prozdrowotną, przeciwdziałaniu osamotnieniu oraz rozwijające indywidualne zainteresowania i aktywność. Zostanie zrealizowanych 798h zajęć. b) wyjazdy na basen, do kina i teatru. Zostanie zrealiz. 25 wyjazdów. c) festyny integracyjne, wieczory taneczno-muzyczne oraz spływy kajakowe. Proj. realizowany jest w odpowiedzi na zdiagn. potrzeby w zakresie wsparcia osób niesamodzielnych, starszych i z niepełnospr. z terenu Gminy Nowa Sucha. Wskaźniki: a) produktu: -Liczba osób objętych szkoleniami/doradztwem w zakresie kompetencji cyfrowych-20. -Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych usługami społecznymi świadczonymi w interesie ogólnym w programie-60. -Liczba osób zagroż. ubóstwem lub wykl. społecznym objętych usługami asystenckimi i opiekuńczymi świadczonymi w społeczności lokalnej w programie -60. -Liczba podmiotów wykorzystujących technologie inform.-komunikacyjne (TIK)-1. b) rezultatu: -Liczba wspartych w programie miejsc świadczenia usług społ., istniejących po zakończeniu proj.-60. -Liczba utw. w progr. miejsc świadczenia usług asystenckich i opiek. istniejących po zakoń. proj.-60. Creation of a daily form of care services in the form of Senior Club As part of the task, the new Senior Club will be equipped and equipped with the premises in the GOKSiR building in Nowa Sucha, in which the new Senior Club will be created. 2. The current functioning of the Senior Club. For project participants, the following are planned: activities aimed at health-promoting activities, preventing loneliness and developing individual interests and activities. 798 hours of classes will be realised. b) trips to the pool, cinema and theatre. It'll be real. 25 trips. c) integration festivities, dance and music evenings and canoeing trips. The project is implemented in response to the needs in terms of support for dependent, elderly and disabled persons from the area of Nowa Sucha commune.
