Cel: Zwiększenie potencjału społeczno-zawodowego 40 os. korzystających z pomocy w tym: 15 os. z niepełnosprawnością i 16 os.III fil pom.oraz ich rodzin, zagrożonych wykluczeniem społecznym zam. na terenie woj. kujawsko-pom, pow. aleksandrowskim, gminy Koneck. poprzez zastosowanie aktywnych form w okresie od 1.07. 2018 r. do 30.11. 2019 r. W ramach projekt. usługi aktywnej integracyjnej będą miały charakter wsparcia indywidu. adresowane dla os., rodzin, oraz do określ. środowiska. Uczestnicy otrzymają wsparci. prowadzące do kompleksowej aktywizacji społeczno-zawodowej poprzez zastosowanie 4 instrumentów aktywnej integracji, wsparcia dla otoczenia oraz działania środowiskowe. Zindywidualizow. i komplek. działania umożliwiające aktywne włączenie społeczne, a także powrót na rynek pracy realizowane będą w oparciu o Prog. Aktyw. Lokal. (PAL). Proces wsparcia os, rodzi i środow. zagrożonych ubóstwem lub wyklucz. społecz. odbywać się będzie w oparciu o ścieżkę reintegr. stworzoną indywidualnie dla każdej os, rodzi , środowi. z uwzględnie. diagnozy społecz, problem, zasob, potencja, predysp i potrzeb. Wskaźnik: Rezultat: Grupa doc.40 os. liczba os. zagrożonych ubóstw. lub wyklucz. społe, które uzyskały kwalifi. po opuszczeniu programu 11 os.(6K/5M), liczba os. zagroż. ubóst. lub wyklucz. społe. poszukują. pracy po opuszczeniu programu 23 os (15K/8M), liczba os. zagroż. ubóst. lub wyklucz. społe. pracujących po opuszczeniu programu(łącznie z pracą na własny rach) 10 os.
