Cílem projektu je podpořit přípravu realizace transformačního plánu a podpořit transformační proces Domova u studánky - koordinace a podpora procesů přechodové fáze transformace zařízení.Obsahem projektu je ucelený soubor provázaných aktivit, které podporují hlavní oblasti realizace transformace - od revize/aktualizace transform. plánu, zpracování dokumentace, komunikační strategii, přípravu realizace přechodů klientů, až po osobní podporu pracovníků a evaluaci.
