Realizacja od 01.01. 2018 do 31.12.2020r. PDW powstanie w Zielonkach, o zasięgu gminnym. Obejmie 120 dziecimłodzieży do 18 r.ż oraz min. 100 ich rodzicówopiekunów prawnych mających trudności w wypełnianiu funkcji opiekuńczowychowawczych. Celem projektu jest poprawa społecznego i emocjonalnego funkcj. dzieci i młodzieży oraz ich rodzicówopiekunów. W ramach proj. 120 dziecimłodzież przejdzie przez diagnozę potrzeb i w oparciu o Indywidualny Plan Rozwoju otrzyma wsparcie w postaci zajęć w formieopiekuńczej tj. pomoc w nauce, zabawę, sport i opiekę oraz rozwijanie pasji i zainteresowań, w tym wycieczki i wyjścia. 120 UP otrzyma również wsparcie w formie specjalistycznej, w tym ok. 20 UP stałą terapięwsparcie. Dodatkowo w PDW zostaną włączeni rodzice, którzy będą mogli ukończyć szkołę rodziców i terapię rodzin. PDW, która będzie funkcjonowała jeszcze przez okres 3 lat po zakończeniu projektu. (Polish)

Implementation from 01.01. 2018 to 31.12.2020 PDW will be built in Zielonki, with communal reach. It will cover 120 children by 18 and at least 100 of their legal parents having difficulties in carrying out caring functions. The aim of the project is to improve the social and emotional function of children and young people and their parents. As part of the project. 120 children will go through a diagnosis of needs and, based on the Individual Development Plan, will receive support in the form of childcare activities, i.e. assistance in learning, fun, sport and care, as well as developing passions and interests, including excursions and departures. 120 UP will also receive support in a specialised form, including ca. 20 UP constant support therapy. In addition, parents will be included in the PDW, who will be able to graduate from their parents’ school and family therapy. PDW, which will continue to function for 3 years after the completion of the project. (English)
