Cel główny projektu: aktywizacja społeczno-zawodowa 34 osób wykluczonych lub zagrożonych wykluczeniem społecznym z powiatu zwoleńskiego w okresie od 01.07.2019 do 30.11.2020 r. Uczestnikami projektu (UP) będzie 34 osoby zagrożone ubóstwem lub wykluczeniem społecznym (17K,17M). W skład grupy docelowej wchodzić będą następujące kategorie UP: a) osoby z niepełnosprawnościami - 12 osób (6K,6M); b) osoby umieszczone w pieczy zastępczej zagrożone ubóstwem lub wykluczeniem społecznym na etapie usamodzielniania lub przygotowania do usamodzielniania tj. osoby powyżej 18 roku życia - 8 osób (4K,4M); c) osoby zagrożone ubóstwem lub wykluczeniem społecznym korzystający ze wsparcia GOPS w Policznej - 14 osób, w tym OzN (7K,7M). Zadania projektowe: 1.Praca socjalna i koordynacja rodzinnej pieczy zastępczej wraz z przeprowadzeniem diagnozy sytuacji problemowej, opracowaniem Indywidualnych Ścieżek Reintegracji i wypłatą świadczeń pieniężnych 2. Usługi aktywnej integracji o charakterze społecznym 3. Usługi aktywnej integracji o charakterze zdrowotnym 4. Usługi aktywnej integracji o charakterze edukacyjnym 5. Usługi aktywnej integracji o charakterze zawodowym 6. Działania o charakterze środowiskowym Główne wskaźniki: Wskaźniki rezultatu: Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym, które uzyskały kwalifikacje lub nabyły kompetencje po opuszeniu programu - 20; Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym, poszukujących pracy po opuszczeniu programu - 10; Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym, pracujących po opuszczeniu programu (łącznie z pracującymi na własny rachunek) - 4; Wskaźniki produktu: Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych wsparciem w programie - 34; Liczba osób z niepełnosprawnościami objętych wsparciem w programie - 12; Miejsce realizacji projektu: powiat zwoleński Okres realizacji projektu: 01.07.2019 r - 30.11.2020 r. Main objective of the project: the socio-occupational activation of 34 persons excluded or threatened with social exclusion from the liberation district in the period from 01.07.2019 to 30.11.2020. The participants of the project (UP) will be 34 people at risk of poverty or social exclusion (17K,17M). The target group will consist of the following UP categories: persons with disabilities – 12 persons (6K,6M); persons placed in foster care at risk of poverty or social exclusion at the stage of independence or preparation for independence i.e. persons over the age of 18-8 persons (4K,4M); C) People at risk of poverty or social exclusion benefiting from GOPS support in Poland – 14 people, including OZN (7K,7M). Project tasks: 1. Social work and coordination of family foster care together with diagnosis of the problem situation, development of Individual Reintegration Paths and payment of cash benefits 2. Active inclusion services of a social nature 3. Active inclusion services of a health nature 4. Active inclusion services of an educational nature 5. Active inclusion services of a professional nature 6. Environmental activities Main indicators: Result indicators: The number of people at risk of poverty or social exclusion who have acquired qualifications or competences after leaving the programme – 20; Number of people at risk of poverty or social exclusion, seeking work after leaving the programme – 10; Number of people at risk of poverty or social exclusion working after leaving the programme (including self-employed) – 4; Output indicators: Number of people at risk of poverty or social exclusion supported in the programme – 34; Number of persons with disabilities supported in the programme – 12; Place of implementation of the project: the Period of implementation of the project: 01.07.2019 r – 30.11.2020 (English)
