Celem głównym projektu jest poprawa dostępności do usług zdrowotnych poprzez realizację programu indywidualnej opieki pielęgniarki środowiskowej w warunkach domowych dla 25 (13K i 12M) mieszkańców gminy Szczerców. Rezultatem projektu będzie wzrost liczby wspartych w programie miejsc świadczenia zdeinstytucjonalizowanych usług zdrowotnych. Osiągnięte to zostanie dzięki realizacji następujących zadań: 1. Kwalifikacja do projektu. 2. Objęcie 25 pacjentów opieką pielęgniarską oraz rehabilitacyjną. 3. Wsparcie dla opiekunów faktycznych osób niesamodzielnych. Projekt skierowany będzie do 25 osób w wieku 50+ zamieszkujących Gminę Szczerców, które zostaną objęte wsparciem w postaci indywidualnej opieki pielęgniarskiej i rehabilitacji w warunkach domowych oraz ich rodzin objętych wsparciem w postaci szkoleń z zakresu opieki nad osobami zależnymi (33 osoby – 17K i 16M). Łącznie z różnych form wsparcia skorzysta 58 os (30K i 28M). Wskaźniki dla projektu: • Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych usługami zdrowotnymi świadczonymi w interesie ogólnym w programie – 25; • Ilość rodzin niesamodzielnych wsparta przez szkolenia – 25; • Liczba wspartych w programie miejsc świadczenia usług zdrowotnych – 8 • Liczba osób z otoczenia objęcia wsparciem w projekcie – 33 • Liczba wspartych w programie miejsc świadczenia usług zdrowotnych istniejących po zakończeniu projektu – 7 (Polish)

The main objective of the project is to improve accessibility to health services through the implementation of an individual care programme for environmental nurses at home for 25 (13K and 12M) residents of the municipality of Szczerców. The project will result in an increase in the number of deinstitutionalised health services supported by the programme. This will be achieved through the following tasks: 1. Qualification for the project. 2. Taking 25 patients into nursing and rehabilitation care. 3. Support for caregivers of real-time dependants. The project will be addressed to 25 people aged 50+ who will be supported in the form of individual nursing care and rehabilitation in the home environment and their families supported in the form of training in the field of care for dependents (33 persons – 17K and 16M). A total of 58 people (30K and 28M) will benefit from various forms of support. Project indicators: • The number of people at risk of poverty or social exclusion covered by health services of general interest in the programme – 25; • Number of dependent families supported by training – 25; • Number of health services supported in the programme – 8 • Number of people from the support environment in the project – 33 • Number of health services supported in the programme after completion of the project – 7 (English)
