Celem projektu jest rozwój deinstytucjonalizacji opieki nad osobami niesamodzielnymi w środowisku lokalnym w wyniku utworzenia Dziennego DomuOpieki Medycznej w Gminie Pałecznica dla 72 osób w tym dla 44 osób starszych 65 zamieszkałych na terenie powiatu proszowickiego jak również wsparcie 72 opiekunów nieformalnych osób niesamodzielnych;Projekt przyczynia się do realizacji celu poprzez zwiększenie dostępności usług medycznych oraz zmianę struktury zapewniania usług med. os. szczególnie powyżej 65 r.ż. tj. deinstytucjonalizację opieki nad os. zależnymi świadczonej w warunkach zbliżonych do domowych.Grupa docelowa:72 os, w tym 43 kobiet 59,5 korzystających z opieki w DDOM, dalej zwanymi pacjentami zamieszkujący w roz. KC na terenie obszaru pow.proszowickiegi, w tym co najmniej 44os. w wieku 65 29K,15M, które: są bezpośrednio po hospitalizacji i ich stan zdrowia wymaga wzmożonej opieki pielęgn, nadzoru nad terapią farmak, kompleksowych działań uspraw. I przeciwdziałających postępującej niesamodzielności oraz wsparcia i eduk. zdr. w okresie ostatnich 12 mc udzielone zostały świadczenia zdrowotne z zakresu leczenia szpitalnego.opiekunowie nieformalni osób niesamodzielnych;DDOMZadania1.
