Cel projektu: Wzrost wiedzy w zakresie zdrowego funkcjonowania rodziny przyczyniający się do aktywnego udziału w życiu społecznym i powrocie na rynek pracy oraz ułatwiający odejście od opieki instytucjonalnej 158 osób (min 50 rodzin) poprzez ułatwienie dostępu do przystępnych cenowo, trwałych oraz wysokiej jakości usług, w tym opieki zdrowotnej i usług socjalnych świadczonych w interesie ogólnym w okresie od 01.01.2019 do 31.08.2019 W projekcie zaplanowano realizację zadań: w ramach USŁUG AKTYWIZACYJNO-INTEGR: 1-indywidualne poradnictwo specjalistyczne 2-zajęcia w ramach Akademii Rodziny 3- wsparcie Konsultanta Rodziny, 4-warsztaty integracji społecznej w ramach USŁUG WSPIERAJĄCYCH INTEGR RODZIN 1- aktywność na rzecz społeczności lokalnej Grupę docelową w projekcie stanowi 158 osób, w tym 90 osób zagrożonych ubóstwem/wykluczeniem społecznym podzielonych na 6 grup po 15 osób. W ramach projektu wsparciem w postaci zajęć opiekuńczo - wychowawczych objętych będzie również 60 do 15 r.ż uczestników projektu (Polish)

Project objective: Increased knowledge of healthy living conditions for families contributing to active participation in and reintegration into the labour market, and facilitating the transition from institutional care of 158 people (min 50 families) by facilitating access to affordable, sustainable and quality services, including health care and social services of general interest, between 01.01.2019 and 31.08.2019. within the framework of INTRA-INOC services: 1-Individual counselling 2-activities within the Academy of the Family 3- support for the Family Consultation, 4-Social Inclusion workshop in the framework of services supporting the RODZIN 1- activity for the local community, the target group in the project is 158 people, including 90 people at risk of poverty/social exclusion divided into 6 groups of 15 people. In the framework of the project, the support in the form of caring activities — bringing up 60 to 15 project participants — will also be supported. (English)
