Celem projektu jest zmniejszenie liczby osób i rodzin zagrożonych wykluczeniem społecznym poprzez wdrożenie dodatkowych usług społecznych na terenie obszaru zdegradowanego (Dolne Miasto/Plac Wałowy/Stare Przedmieście) charakteryzującego się dużym natężeniem problemów społecznych. Grupą docelową projektu są rodziny dysfunkcyjne niewydolne wychowawczo, ze szczególnym uwzględnieniem dzieci i młodzieży wychowującej się w tych rodzinach. Preferencję uczestnictwa będą miały osoby/rodziny korzystające z PO Pomoc Żywnościowa. Wsparcie uczestników projektu będzie miało charakter zindywidualizowanych i kompleksowych usług w środowisku lokalnym. Lokalny system wsparcia oparty będzie o placówkę wsparcia dziennego oraz centrum wsparcia i terapii rodzin. Wszyscy uczestnicy zostaną objęci wszechstronną diagnozą, na podstawie której opracowane zostaną indywidualne ścieżki reintegracji. Każdy z uczestników skorzysta z tych form wsparcia, których najbardziej potrzebuje. Usługi dostępne w ramach projektu to m.in. nowe miejsca w placówce wsparcia dziennego, zajęcia edukacyjne, zajęcia profilaktyczne, wsparcie psychologiczno-pedagogiczne, psychoterapia, trening umiejętności społecznych, interwencja kryzysowa, animacja społeczności lokalnej, wolontariat. Efektem realizacji projektu będzie 11 trwałych miejsc świadczenia usług społecznych oraz wsparcie udzielone 75 osobom zagrożonym ubóstwem i/lub wykluczeniem społecznym, w tym 15 osobom poszukującym pracy. Projekt jest zgodny ze Strategią ZIT. (Polish)

The aim of the project is to reduce the number of people and families at risk of social exclusion by implementing additional social services in the degraded area (Lower Town/Square/Old Town), characterised by a high degree of social problems. The target group of the project is dysfunctional inefficient families, with a particular focus on children and young people bringing up those families. Preference will be given to persons/families benefiting from the OP Food Aid. Support to the project participants will be individualised and complex services in the local environment. The local support system will be based on the daily support facility and the family support and treatment centre. All participants will be subject to a comprehensive diagnosis, on the basis of which individual pathways will be developed. Each participant will benefit from the support it needs most. The services available under the project include, inter alia, new places in the daily support facility, educational activities, prevention activities, psycho-social support, psychotherapy, social skills training, crisis intervention, community animation, voluntary activities. The project will deliver 11 sustainable social services and support 75 people at risk of poverty and/or social exclusion, including 15 job seekers. The project is in line with the ITI strategy.
