Celem głównym projektu jest zwiększona dostępności do wysokiej jakości świadczeń zdrowotnych realizowanych w ramach podstawowej opieki zdrowotnej (POZ) przez NZOZ "Zdrowie" s.c. Izabela Dorota Anasiewicz-Kostrzewa, Maria Kokoszka w Janowie Lubelskim. Przedmiot projektu obejmuje rozszerzenie i uzupełnienie zakresu diagnostyki obrazowej realizowanej w POZ poprzez zakup przenośnego ultrasonografu wraz z niezbędnym osprzętem komputerowym (laptop, przenośna drukarka). Katalog wydatków uzupełniają: opracowanie Studium Wykonalności i działania promocyjne. Realizacja inwestycji pozwoli na wykonywanie badań USG przez lekarza POZ podczas wizyty w domu pacjenta. Ma to kluczowe znaczenie dla tych osób, które ze względu na stan zdrowia, niepełnosprawność czy podeszły wiek (grupa osób zależnych) nie mają możliwości skorzystania ze stacjonarnych form diagnostyki. Inwestycji wykazuje spójność z założeniami Narzędzia 14 Policy Paper dla ochrony zdrowia jak również zapisami map potrzeb zdrowotnych w obszarze chorób: nowotworowych, kardiologicznych, gruczołów wydzielania wewnętrznego, cukrzycy, nowotworów niezłośliwych, metabolicznych, układu moczowo - płciowego u mężczyzn i kobiet, wątroby i trzustki. Kluczowe wskaźniki realizacji projektu to: Ludność objęta ulepszonymi usługami zdrowotnymi (CI36) - 12 138 osób; Liczba wspartych podmiotów leczniczych - 1szt.; Nakłady inwestycyjne na zakup aparatury medycznej - 91 666,67 zł. Grupę docelową projektu stanowią pacjenci NZOZ. Projekt ma charakter nieinfrastrukturalny (w rozumieniu procedur OOŚ). (Polish)

The main objective of the project is to increase the availability of health services under the primary health care (POZ) by NZOZ “Health”, “Health” s.c. Izabela Dorota Amediacz-Kwarned, Maria Kooszka, Janowie Lubelskim. The object of the project includes the extension and completion of an imaging diagnosis carried out in the POW by the purchase of a portable ultrasound with the necessary computer equipment (laptop, printer). The expenditure catalogue is complemented by: development of a feasibility study and promotional activities. The realisation of the investment will allow the USG to be tested by the POZ doctor during his visit to the patient’s home. This is crucial for those who, because of their state of health, disability or old age (a group of dependent persons) do not have the possibility to use stationary forms of diagnosis. The investments are consistent with the objectives of the Health Paper 14 Policy Paper, as well as the health needs mapping in the area of disease: cancer, cardiological, diabetes, endogenous diabetes, metabolic, metabolic, urinary tract, male and female, liver and pancreas. The key performance indicators for the project are: The population covered by the improved health services (CI36) — 12 138 people; Number of medical entities supported — 1 t.; Investment in the purchase of medical equipment — PLN 91 666,67. The target group of the project is the patients of the NZOZ. The project is not an infrastructure (within the meaning of the EIA procedures). (English)
