Projekt skierowany jest do ogółem 40 osób (22K, 18M) zagrożonych ubóstwem lub wykluczeniem społecznym z terenów powiatów wejherowskiego i puckiego, znajdujących się w trudnej sytuacji materialnej, bezrobotnych i/lub biernych zawodowo, w tym 6 osób z niepełnosprawnościami. Celem projektu jest kompleksowe wsparcie ww. osób zmierzające ostatecznie do zwiększenia ich zatrudnienia. Cele szczegółowe to wywołanie następującej zmiany u beneficjentów: uzyskanie kwalifikacji zawodowych, podjęcie zatrudnienia lub poszukiwanie pracy, aktywizacja społeczna i zawodowa. Projekt realizowany jest przez Fundację Phenomen i Partnera w projekcie Powiatowe Centrum Pomocy Rodzinie w Pucku. Działania w ramach projektu rozpoczną się od opracowania dla każdego beneficjenta ścieżki reintegracji w ramach której określone zostaną indywidualne dla każdej osoby co do formy i intensywności wsparcia narzędzia, takie jak m.in.: poradnictwo psychologiczne, poradnictwo prawne, wsparcie pracownika socjalnego, brokera edukacyjnego, zajęcia ukierunkowane na rozwój zainteresowań i aspiracji edukacyjnych, wsparcie doradcy zawodowego, pośrednictwo pracy, staże zawodowe, kursy i szkolenia zawodowe, opieka dla osób zależnych, rehabilitacja zdrowotna osób niepełnosprawnych. Planowane działania, obejmujące również zwroty kosztów dojazdu, stypendia stażowe czy catering, mają zapewnić kompleksowe wsparcie beneficjentów dostosowane do ich zindywidualizowanych potrzeb i oczekiwań. Rezultatem projektu będzie osiągnięcie efektywności społecznej na poziomie co najmniej 34% w odniesieniu do całej grupy beneficjentów oraz efektywności zatrudnieniowej na poziomie co najmniej 22% w odniesieniu do osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych wsparciem w programie i na poziomie 12% w odniesieniu do osób o znacznym stopniu niepełnosprawności, osób z niepełnosprawnością intelektualną i osób z niepełnosprawnościami sprzężonymi. Minimum 40% uczestników uzyska nowe kwalifikacje zawodowe. (Polish)

The project is aimed at a total of 40 persons (22K, 18M) at risk of poverty or social exclusion from the areas of the districts of Europe’s entry into and out of all difficult economic situations, the unemployed and/or the inactive, including 6 persons with disabilities. The aim of the project is to provide comprehensive support for the above-mentioned persons with a view ultimately to increasing their employment. The specific objectives are to have the following change in the beneficiaries: acquisition of professional qualifications, employment or job search, social and professional activation. The project is implemented by the Phenomen and Partnera Foundation in the project of the District Centre for Family Assistance in Puck. Project activities will start with the development of a reintegration pathway for each beneficiary to identify individuals for each individual in the form and intensity of the support tools, such as: psychological counselling, legal advice, support to a social worker, an education broker, activities aimed at developing interests and educational aspirations, supporting professional counsellors, placement of work, job placements, vocational courses and training, care for dependent persons, and rehabilitation of the health of persons with disabilities. The planned actions, including travel expenses, fellowships and catering, are intended to provide comprehensive support to beneficiaries tailored to their personalised needs and expectations. The project will result in a social efficiency of at least 34 % for the entire group of beneficiaries and the employment efficiency of at least 22 % for those at risk of poverty or social exclusion supported by the Programme and at a level of 12 % for severely disabled persons, persons with intellectual disabilities and those with secondary disabilities. A minimum of 40 % of participants will gain new professional qualifications. (English)
