Cel główny: wzrost dostępn do wysokiej jakości usł. społ. dostosowanych do indywidualnych potrzeb 31 osób (24K, 7M) zagroż ubóstwem i wykl społ. z terenu Gminy Kodeń (GK) do 30.04.2023 r. poprzez utw i prowadz zajęć w ramach 2 KS w Kostomłotach II i Dobromyślu oraz zorganizowania uług asystenckich dla 5 mieszkańców GK. Grupę docelową (GD) P stanowi 31 os. (24K, 7M) wyklucz lub zagrożonych ubóstwem i wykl społ, (jako najbardziej potrzeb kompleks wsparcia zgodnie z przepr diagnozą), w tym 6 os. niepełnosprawnych, które spełniają nast. warunki form.: - są mieszkańcami GK (w rozumieniu przepisów KC). - zamieszkują ter. wiejskie, - mają ukończone 60 lat (dot. Klubów Seniora) - mają status os. niepełnosprawnej (dot. usł. asystenckich). Główne wskaźniki -Liczba wspartych w programie miejsc świadcz usł społ, istniejących po zakończeniu projektu: 27 -Liczba utworzonych w programie miejsc świadcz usł asyst i opiekuńczych istniejących po zakończeniu P: 27 -Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych usługami społecznymi świadczonymi w interesie ogólnym w programie: 31 -Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych usługami asystenckimi i opiekuńczymi świadczonymi w społeczności lokalne w programie: 31 Zadania: 1. Organizacja i funkcjonowanie Klubów Seniora w Gminie Kodeń; 2. Zajęcia cykliczne prowadzone w ramach 2 Klubów Seniora w Gminie Kodeń 3. Main objective: the increase in access to high-quality social services tailored to the individual needs of 31 people (24K, 7M) at risk of poverty and society from the area of the Commune of Kodeń (GK) until 30.04.2023 through the establishment and running of classes within the framework of 2 KS in Kostomłoty II and Dobromyśl and organising assistant services for 5 inhabitants of the GK. The target group (GD) P is 31 pers. (24K, 7M) exclude or at risk of poverty and society, (as the most necessary support complex according to the diagnosis), including 6 persons with disabilities, which meet the following conditions of form: they are residents of the GK (within the meaning of the provisions of the CC). they live in rural areas, — they are over 60 years old (dot. Senior clubs) — they have the status of a disabled person (about assistant services). Main indicators -Number of places supported in the programme provide social services, existing after the project is completed: 27 -Number of places created in the programme provide assistance and care services existing after the completion of P: 27 — Number of persons at risk of poverty or social exclusion covered by social services of general interest in the programme: 31 — Number of people at risk of poverty or social exclusion covered by assistance and care services provided in local communities in the programme: 31 Tasks: 1. Organisation and operation of Senior Clubs in the Commune of Code; 2. Cyclical classes conducted as part of 2 Senior Clubs in the Commune of Code 3.
