Celem głównym projektu jest wzrost dostępności do wysokiej jakości usług społ. w Gm. Leśniowice dla osób wyklucz. lub zagroż. ubóstwem i wyklucz. społ. , w tym zwiększenie dostępu do usług opiekuńczych poza miejscem zam. w formie DDP (20os), w miejscu zam. (10 os.) z wykorzystaniem teleopieki (10 najbardziej potrzebujących osób z 30 osób objętych wsparciem w/w) oraz poprzez wsparcie op. faktycznych (10os.) w okresie I.2021-VIII.2023r. Wsparciem w projekcie zostanie objętych 30 osób (28K, 2M) wykluczonych lub zagrożonych ubóstwem i wykluczeniem społecznym (w tym 20 osób (20K) wykluczonych lub zagrożonych ubóstwem (…) w ramach DDP i 10 osób (8K, 2M), które korzystać będą ze specj. usług opiek. świadczonych w m. zam.) oraz 10 osób (6K, 4M) z otoczenia osób wykluczonych społecznie, których udział w projekcie jest niezbędny dla skutecznego wsparcia osób zagrożonych ubóstwem lub wykluczeniem społecznym (op. faktyczni)., które objęte zostaną wsparciem w ramach warsztatów z psychologiem. Zad.: - Utw DDP (II.2021-VIII.2021- zakupy i prace ad.-rem.; b.f. II.2021-VIII.2023r); - Zapewn bież funkc. DDP (II.2021-VIII.2023r), w tym zatrudnienie 2 Opiekunów w DDP - Wsparcie dla 10 OF (2021r) i świadcz specj usług opiek w m. zamieszk dla 10os. (II.2021-VIII.2023r); - Świad. usł. teleopieki (zakup, aktyw. i wdroż VI.2021-IX.2021; funk. IX.2021-VIII.2023r). W-ca osiągnie cel Programu, wyrażony m.in. wskaźnikami: - L. OzN objętych wsparciem w programie – 10os.; - L. os pochodzących z obsz wiejskich -30os.; - L. utw w programie miejsc świadcz usług asyst i opiek istniejących po zakończ proj -21. Proj. przyczyni się do real. celu szczeg. dział. 11.2 RPO WL 2014-2020: „wzrost dostęp usług społ i zdrow poprzez cele szczeg. proj. (tj. ww. zad). Proj. będzie real. zg. z „Wyt. w zakr. real. przed w obsz. Wł. społ. i zwalczania ub z wykorzyst. śr. The main objective of the project is to increase the availability of high-quality social services in Leśniowice for people excluded or at risk of poverty and social exclusion. including increasing access to care services outside the premises in the form of DDP (20os), in the place of the castle. (10 pers.) using telecare (10 people most in need out of the 30 persons supported by the above) and through the support of op. de facto (10 persons) during the period I.2021-VIII.2023. The project will support 30 people (28K, 2M) excluded or at risk of poverty and social exclusion (including 20 people (20K) excluded or at risk of poverty (...) within the framework of DDP and 10 persons (8K, 2M) who will benefit from special care services provided in Poland) and 10 people (6K, 4M) from the environment of socially excluded people whose participation in the project is necessary to effectively support people at risk of poverty or social exclusion (op. actual). Wow.: — UTW DDP (II.2021-VIII.2021 — purchases and works ad-rem.; b.f. II.2021-VIII.2023r; — Ensure the running of funkc. DDP (II.2021-VIII.2023), including employment of 2 Caregivers in DDP — Support for 10 OF (2021) and providing special care services in housing for 10 people. (II.2021-VIII.2023); — Certificate of telecare services (purchase, assets and implementation VI.2021-IX.2021; it’s Funk. IX.2021-VIII.2023). W-ca will achieve the Programme’s objective, expressed, inter alia, by indicators: — L. OzN supported by the programme — 10 persons; L. wasps coming from rural areas -30os.; — L. utw in the programme of places provide assistance and care services existing after the completion of Project -21. The project will contribute to the real purpose of the section. 11.2 ROP WL 2014-2020: “enhancement of access to social and health services through specific objectives of project.
