    By 2021, mPower aims to stimulate transformation in older people’s services in Ireland, Northern Ireland and Scotland, enabling people to live well, safely and independently in their own homes, supported by a modernised infrastructure for healthy ageing. mPower will champion a preventative approach to care, supporting societal change by empowering more people to self-manage their health and care in the community. The project will result in improved health and wellbeing for citizens, delivering 7,000 additional episodes of health, community and social care on a cross border basis. Key project objectives are to: 1. Establish cross sectoral collaboration across Ireland, Northern Ireland and Scotland to support greater mobility, confidence and reach of health and care professionals and services. 2. Support 2500 people to self-manage their health and wellbeing more effectively in the community; 3. Deliver 4500 eHealth interventions increasing awareness of their benefits in supporting sustainable service delivery. 4. Deliver training programmes to 420 health and care providers. 5. Effectively engage individuals, community organisations and services in redesign of health and care services. The project will principally target citizens in the 65+ older people at risk age group. A new, cross border mPower service will be established to deliver outputs across three participating countries. The service will be a hub and spoke construct, with a core unit and 7 local deployment teams. The cross border services developed will deliver: • A Community Navigator Service undertaking home visits, co-producing 2500 personalised Wellbeing Plans with citizens, focused on prevention and self-management. • 4500 episodes of eHealth interventions through use of Home and Mobile Health Monitoring solutions, Digital Health and Wellbeing Services/Apps and Video Enabled Care Services.
