В рамките на проекта се предвижда осигуряване на достъп до интегрирани услуги, за социално включване в общността или в домашна среда, в т.ч. подкрепящи и интегрирани между секторни услуги, според специфичните потребности на човека в неравностойно положение въз основа на индивидуална социална оценка чрез подкрепа и разширяване на дейността на съществуващото звено за почасово предоставяне на услуги, създадено по проект "Помощ в дома". В звеното ще се представят по иновативен начин на нуждаещи се лица, живеещи на територията на община Копривщица три типа почасови услуги в домашна среда – за лична помощ, за социална подкрепа и помощ при комунално-битови дейности, както и допълнителни здравни и социални услуги в среда различна от домашната, а именно: здравни услуги, рехабилитационни услуги, мотивационни и/или психологически услуги на нуждаещите се лица за срок от 18 месеца. Към него ще бъдат назначени по трудови договори 24 души персонал, които ще предоставят услуги за 38 потребители. / В това число персонала и потребителите от проект "Нови алтернативи", които ще бъдат включени след 01.03.2016год/.Настоящето проектно предложение на община Копривщица отговаря на общата и специфичните цели на схема „Независим живот” по следния начин:-представителите на целевата група ще пулучат достъп до дългосрочна здравно-социална грижа, вкл. и достъп до заетост, според специфичните потребности на човека в неравностойно положение на основата на индивидуална оценка на потребностите;- създава се устойчив децентрализиран модел на предоставяне на социални услуги и осигуряване на условия за достоен живот за лица, които са или се намират в риск от социална изолация;-предоставя възможности за връщането на реалния пазар на труда на лицата, които полагат грижи за близките си с увреждания. The project envisages providing access to integrated services, for social inclusion in the community or in the home environment, including supporting and integrated between sectoral services, according to the specific needs of disadvantaged people on the basis of an individual social assessment by supporting and expanding the activities of the existing unit for hourly service delivery established under the project “Help at home”. The unit will present in an innovative way to deprived persons living on the territory of the municipality of Koprivshtitsa three types of hourly services in the home environment — for personal assistance, for social support and assistance in utility activities, as well as additional health and social services in an environment other than the home environment, namely: health services, rehabilitation services, motivational and/or psychological services to people in need for a period of 18 months. It will be recruited under employment contracts 24 staff who will provide services to 38 consumers. /Including the staff and users of the project “New Alternatives”, which will be included after 01.03.2016/.The present project proposal of the Municipality of Koprivshtitsa meets the general and specific objectives of the “Independent Life” scheme in the following way:-the representatives of the target group will have access to long-term health-social care, including access to employment, according to the specific needs of the disadvantaged person on the basis of an individual assessment of needs;- creates a sustainable decentralised model of provision of social services and decent living conditions for persons who are or are at risk of social exclusion;- provide opportunities for the return of the real labour market to the persons taking care of their close disabilities. Le projet prévoit de fournir un accès à des services intégrés, à des fins d’inclusion sociale dans la communauté ou dans l’environnement domestique, y compris un soutien et une intégration entre les services sectoriels, en fonction des besoins spécifiques des personnes défavorisées, sur la base d’une évaluation sociale individuelle, en soutenant et en élargissant les activités de l’unité existante pour la prestation de services horaires établie dans le cadre du projet «Aide à domicile». L’unité présentera de manière novatrice aux personnes démunies vivant sur le territoire de la municipalité de Koprivshtitsa trois types de services horaires dans l’environnement familial — pour l’assistance personnelle, l’aide sociale et l’assistance dans le cadre d’activités de services publics, ainsi que des services sanitaires et sociaux supplémentaires dans un environnement autre que le milieu familial, à savoir: services de santé, services de réadaptation, services de motivation et/ou psychologiques destinés aux personnes dans le besoin pendant une période de 18 mois. Il sera recruté dans le cadre de contrats de travail 24 agents qui fourniront des services à 38 consommateurs. /Incluant le personnel et les utilisateurs du projet «Nouvelles alternatives», qui sera inclus après le 01.03.2016/.La présente proposition de projet de la municipalité de Koprivshtitsa répond aux objectifs généraux et spécifiques du régime «Vie Indépendante» de la manière suivante:-les représentants du groupe cible auront accès à des soins de santé et à des soins sociaux à long terme, y compris l’accès à l’emploi, en fonction des besoins spécifiques de la personne défavorisée sur la base d’une évaluation individuelle des besoins;- créer un modèle décentralisé durable de prestation de services sociaux et de conditions de vie décentes pour les personnes qui sont ou sont menacées d’exclusion sociale;- fournir des possibilités de retour du marché réel du travail aux personnes qui prennent soin de leur handicap proche. Das Projekt sieht vor, den Zugang zu integrierten Dienstleistungen für die soziale Inklusion in der Gemeinschaft oder im häuslichen Umfeld zu ermöglichen, einschließlich der Unterstützung und Integration sektoraler Dienstleistungen entsprechend den spezifischen Bedürfnissen benachteiligter Menschen auf der Grundlage einer individuellen sozialen Bewertung durch Unterstützung und Ausweitung der Tätigkeiten des bestehenden Referats für die Stundendienstleistung, das im Rahmen des Projekts „Help at home“ eingerichtet wurde. Die Einheit wird auf innovative Weise den auf dem Gebiet der Gemeinde Koprivshtitsa lebenden Personen drei Arten von Stundendiensten im häuslichen Umfeld präsentieren – für persönliche Hilfe, für soziale Unterstützung und Unterstützung bei Versorgungstätigkeiten sowie zusätzliche Gesundheits- und Sozialdienste in einem anderen Umfeld als dem häuslichen Umfeld, nämlich: Gesundheitsdienstleistungen, Rehabilitationsleistungen, Motivations- und/oder psychologische Dienstleistungen für Bedürftige für einen Zeitraum von 18 Monaten. Es wird im Rahmen von Arbeitsverträgen 24 Mitarbeiter eingestellt, die 38 Verbrauchern Dienstleistungen erbringen werden. Der vorliegende Projektvorschlag der Gemeinde Koprivshtitsa erfüllt die allgemeinen und spezifischen Ziele des Programms „Unabhängiges Leben“ wie folgt:-Die Vertreter der Zielgruppe haben Zugang zu langfristiger Gesundheits- und Sozialfürsorge, einschließlich des Zugangs zur Beschäftigung, entsprechend den spezifischen Bedürfnissen der benachteiligten Person auf der Grundlage einer individuellen Bedarfsbeurteilung;- schafft ein nachhaltiges dezentralisiertes Modell für die Bereitstellung sozialer Dienstleistungen und menschenwürdige Lebensbedingungen für Personen, die von sozialer Ausgrenzung bedroht sind oder sind;- bieten Chancen für die Rückkehr des wirklichen Arbeitsmarktes für Menschen, die ihre Behinderung aufnehmen. Het project beoogt toegang te bieden tot geïntegreerde diensten, sociale inclusie in de gemeenschap of in de thuisomgeving, met inbegrip van ondersteuning en integratie tussen sectorale diensten, afhankelijk van de specifieke behoeften van kansarmen op basis van een individuele sociale beoordeling door de activiteiten van de bestaande eenheid voor dienstverlening per uur te ondersteunen en uit te breiden die in het kader van het project „Help thuis” is ingesteld. De eenheid zal op een innovatieve manier presenteren aan behoeftigen die op het grondgebied van de gemeente Koprivshtitsa wonen drie soorten uurdiensten in de thuisomgeving — voor persoonlijke bijstand, voor sociale ondersteuning en bijstand bij nutsvoorzieningen, alsmede voor aanvullende gezondheids- en sociale diensten in een andere omgeving dan de thuisomgeving, namelijk: gezondheidsdiensten, revalidatiediensten, motiverende en/of psychologische diensten voor mensen in nood gedurende een periode van 18 maanden. Het zal worden aangeworven op basis van arbeidsovereenkomsten 24 personeelsleden die diensten verlenen aan 38 consumenten. /Met inbegrip van het personeel en de gebruikers van het project „Nieuwe alternatieven”, dat zal worden opgenomen na 01.03.2016/.Het huidige projectvoorstel van de gemeente Koprivshtitsa voldoet op de volgende manier aan de algemene en specifieke doelstellingen van het „Onafhankelijk Leven”-systeem:- de vertegenwoordigers van de doelgroep zullen toegang hebben tot langdurige gezondheidszorg, met inbegrip van toegang tot werk, op basis van een individuele beoordeling van de behoeften van de benadeelde persoon;- creëert een duurzaam gedecentraliseerd model voor het verstrekken van sociale diensten en fatsoenlijke levensomstandigheden voor personen die sociale uitsluiting lopen of dreigen te worden;- biedt kansen voor de terugkeer van de werkelijke arbeidsmarkt van hun zorgverleners. Il progetto prevede l'accesso a servizi integrati, per l'inclusione sociale nella comunità o nell'ambiente domestico, compreso il sostegno e l'integrazione tra i servizi settoriali, in funzione delle esigenze specifiche delle persone svantaggiate sulla base di una valutazione sociale individuale, sostenendo e ampliando le attività dell'unità esistente per la fornitura di servizi orari stabiliti nell'ambito del progetto "Aiuto a domicilio". L'unità presenterà in modo innovativo alle persone indigenti che vivono nel territorio del comune di Koprivshtitsa tre tipi di servizi orari nell'ambiente domestico — per l'assistenza personale, per l'assistenza sociale e per l'assistenza in attività di pubblica utilità, nonché per servizi sanitari e sociali aggiuntivi in un ambiente diverso dall'ambiente domestico, vale a dire: servizi sanitari, servizi di riabilitazione, servizi motivazionali e/o psicologici alle persone bisognose per un periodo di 18 mesi. Sarà assunto con contratti di lavoro 24 agenti che forniranno servizi a 38 consumatori. /Incluso il personale e gli utenti del progetto "Nuove alternative", che sarà incluso dopo il 01.03.2016/.La presente proposta di progetto del Comune di Koprivshtitsa soddisfa gli obiettivi generali e specifici del regime "Independent Life" nel modo seguente:-i rappresentanti del gruppo destinatario avranno accesso all'assistenza sanitaria-sociale a lungo termine, compreso l'accesso all'occupazione, in funzione delle esigenze specifiche della persona svantaggiata sulla base di una valutazione individuale dei bisogni;- crea un modello decentrato sostenibile di prestazione di servizi sociali e condizioni di vita dignitose per le persone che sono o sono a rischio di esclusione sociale;- offrono opportunità per il ritorno del reale mercato del lavoro alle persone che si prestano in stretta assistenza alle persone con disabilità. El proyecto prevé el acceso a servicios integrados, para la inclusión social en la comunidad o en el entorno doméstico, incluido el apoyo e integración entre los servicios sectoriales, de acuerdo con las necesidades específicas de las personas desfavorecidas sobre la base de una evaluación social individual, apoyando y ampliando las actividades de la unidad existente para la prestación de servicios por hora establecida en el marco del proyecto «Ayuda en el hogar». La unidad presentará de manera innovadora a las personas necesitadas que viven en el territorio del municipio de Koprivshtitsa tres tipos de servicios por hora en el entorno doméstico: asistencia personal, apoyo social y asistencia en actividades de servicios públicos, así como servicios sanitarios y sociales adicionales en un entorno distinto del hogar, a saber: servicios de salud, servicios de rehabilitación, servicios motivadores o psicológicos para personas necesitadas durante un período de 18 meses. Se contratará mediante contratos de trabajo a 24 funcionarios que prestarán servicios a 38 consumidores. /Incluye al personal y a los usuarios del proyecto «Nuevas Alternativas», que se incluirá después del 01.03.2016/.La presente propuesta de proyecto del municipio de Koprivshtitsa cumple los objetivos generales y específicos del régimen de «vida independiente» de la siguiente manera:-los representantes del grupo destinatario tendrán acceso a la atención sanitaria y social a largo plazo, incluido el acceso al empleo, de acuerdo con las necesidades específicas de la persona desfavorecidas sobre la base de una evaluación individual de las necesidades;- crea un modelo descentralizado sostenible de prestación de servicios sociales y condiciones de vida dignas para las personas que están o corren el riesgo de exclusión social;- ofrecen oportunidades para el retorno del mercado laboral real a las personas que se encuentran en situación de exclusión social.
