Projekt "Dzienny Dom Opieki Medycznej w Rudzie Śląskiej" to spójna i kompleksowa forma wsparcia skierowana do 75 pacjentów z terenu Rudy Śląskiej. W ramach projektu będą udzielane świadczenia osobom, wymagającym ze względu na stan zdrowia, świadczeń pielęgnacyjnych, opiekuńczych i rehabilitacyjnych oraz kontynuacji leczenia, a niewymagającym hospitalizacji w oddziale szpitalnym, którzy w ocenie skalą poziomu samodzielności (skala Barthel) otrzymali 40-65 punktów. Projekt zakłada realizację w okresie 01.10.2020 - 31.03.2022 następujących zadań: - Adaptacja infrastruktury Dziennego Domu Opieki Medycznej w w Rudzie Śląskiej na potrzeby funkcjonowania DDOM - Funkcjonowanie Dziennego Domu Opieki Medycznej w Rudzie Śląskiej Grupę docelową stanowią osoby wykluczone lub zagrożone ubóstwem lub wykluczeniem społecznym oraz ich otoczenie z miasta Ruda Śląska.
