Przedmiotem projektu jest podjęcie działań skutkujących poprawą stanu zdrowia osób zagrożonych ubóstwem i wykluczeniem społecznym z gminy Korfantów i okolic: osób starszych i niepełnosprawnych. W ramach projektu będą świadczone usługi zdrowotne w środowisku domowym oraz w Opolskim Centrum Rehabilitacji w dziennej formie. Zostanie zorganizowana wypożyczalnia sprzętu rehabilitacyjnego, a także będą organizowane działanie edukacyjne: szkolenia dla opiekunów faktycznych w zakresie opieki medycznej oraz dla kadry medycznej – w celu podniesienia kompetencji i poprawy jakości świadczonych usług. W projekcie preferowane będą osoby ze wskazaniem lekarskim bezpośrednio lub do pół roku po odbytej hospitalizacji, spełniające warunki rekrutacji. W dziennej formie pobytu zostanie utworzone 10 miejsc opieki, a do 6 pacjentów dziennie pracownicy OCR będą dojeżdżać (zostanie uruchomiona grupa wyjazdowa). Projekt stanowi odpowiedź na dostrzeżone problemy społeczne: -przyspieszony proces starzenia się ludności (wzrost liczby jednoosobowych gospodarstw domowych starszych osób samotnie mieszkających o utrudnionym dostępie do usług zdrowotnych) i problem związany ze wzrostem obciążeń ekonomicznych rodzin, -trudność lub niemożność samodzielnego wykonywania podstawowych czynności życiowych (wg wskazań skali Barthel) i konieczność podjęcia działań usprawniających u osób niepełnosprawnych, niesamodzielnych, szczególnie w wieku 65+, -niewielki zakres zabezpieczenia potrzeb starszych mieszkańców w zakresie miejsc pobytu w dziennych domach pomocy przy systematycznym wzroście ich liczby. W efekcie projektu zostanie zintensyfikowana pomoc pielęgniarsko-medyczna, w tym rehabilitacja i fizjoterapia, terapia zajęciowa, będą prowadzone działania, prowadzące do usprawnienia i zwiększenia komfortu życia tych osób. The subject of the project is to take measures to improve the health of people at risk of poverty and social exclusion from the Korfantów commune and the surrounding areas: the elderly and the disabled. The project will provide health services in the home environment and in the Opolskie Centre for Rehabilitation in a daily form. A rental of rehabilitation equipment will be organised and an educational activity will be organised: training for carers in the field of medical care and medical staff – in order to improve competences and improve the quality of services provided. The project will prefer people with medical indications directly or up to six months after hospitalisation, fulfilling the conditions for recruitment. 10 places of care will be created in the daily form of stay, and up to 6 patients per day will be commuted by OCR staff (outbound group will be activated). The project responds to the perceived social problems: —accelerated process of ageing population (increased number of single-person households living alone with difficult access to health services) and the problem of increasing the economic burden of families, -difficulty or inability to perform basic life activities independently (according to the Barthel scale) and the need to take measures to improve disabled persons, especially at 65+, -small coverage of the needs of older residents in terms of living places in daily homes. As a result of the project, nursing and medical assistance will be intensified, including rehabilitation and physiotherapy, occupational therapy, activities will be carried out, leading to improvement and increased comfort of life of these people. L’objectif du projet est de prendre des mesures pour améliorer l’état de santé des personnes exposées au risque de pauvreté et d’exclusion sociale de la municipalité de Korfant et des environs: personnes âgées et handicapées. Le projet fournira quotidiennement des services de santé à domicile et au centre de réadaptation d’Opole. Il y aura une location de matériel de réhabilitation, ainsi qu’une activité éducative: formation des soignants effectifs dans le domaine des soins médicaux et du personnel médical — afin d’améliorer les compétences et la qualité des services fournis. Dans le projet, les personnes ayant une indication médicale seront préférées soit directement, soit jusqu’à un demi-an après l’hospitalisation, répondant aux conditions de recrutement. 10 lieux de soins seront créés dans la forme quotidienne de séjour et jusqu’à 6 patients par jour seront transportés par le personnel de l’OCR (un groupe de départ sera lancé). Le projet répond aux problèmes sociaux perçus: — le processus accéléré du vieillissement (augmentation du nombre de ménages monoparentaux de personnes âgées vivant seules ayant un accès difficile aux services de santé) et le problème de l’augmentation de la charge économique pour les familles, — la difficulté ou l’incapacité de mener de manière autonome des activités de base (comme l’indique l’échelle Barthel) et la nécessité de mesures d’amélioration pour les personnes handicapées et les personnes dépendantes, en particulier à l’âge de 65 ans et plus, — la faible proportion des besoins des personnes âgées en termes de lieux de résidence dans des foyers quotidiens, avec une augmentation systématique de leur nombre. Grâce au projet, les soins infirmiers et l’assistance médicale, y compris la réadaptation et la physiothérapie, l’ergothérapie, seront renforcés, ce qui permettra d’améliorer et d’améliorer le bien-être de ces personnes. Ziel des Projekts ist es, Maßnahmen zur Verbesserung des Gesundheitszustands von von Armut und sozialer Ausgrenzung bedrohten Menschen aus der Gemeinde Korfant und Umgebung zu ergreifen: ältere und behinderte Menschen. Das Projekt wird täglich Gesundheitsdienstleistungen im häuslichen Umfeld und im Opole Rehabilitationszentrum anbieten. Es wird eine Anmietung von Rehabilitationsausrüstung sowie eine pädagogische Aktivität geben: Ausbildung der tatsächlichen Pflegekräfte im Bereich der medizinischen Versorgung und des medizinischen Personals – um die Kompetenz zu verbessern und die Qualität der erbrachten Dienstleistungen zu verbessern. Im Projekt werden Personen mit medizinischer Indikation entweder direkt oder bis zu einem halben Jahr nach dem Krankenhausaufenthalt bevorzugt, um die Bedingungen für die Rekrutierung zu erfüllen. 10 Betreuungsorte werden in der täglichen Form des Aufenthalts geschaffen, und bis zu 6 Patienten pro Tag werden von OCR-Mitarbeitern pendeln (eine Abgangsgruppe wird gestartet). Das Projekt reagiert auf die wahrgenommenen sozialen Probleme: — der beschleunigte Alterungsprozess (Erhöhung der Zahl der allein lebenden Einpersonenhaushalte älterer Menschen mit schwierigem Zugang zu Gesundheitsdiensten) und das Problem der Erhöhung der wirtschaftlichen Belastung der Familien – die Schwierigkeit oder Unfähigkeit, selbständig grundlegende Lebensaktivitäten (wie in der Barthel-Skala angegeben) durchzuführen, und die Notwendigkeit von Verbesserungsmaßnahmen für Menschen mit Behinderungen und abhängige Personen, insbesondere im Alter von 65 Jahren, – das geringe Ausmaß der Sicherung des Bedarfs älterer Bewohner an Wohnorten in Tagesheimen mit einer systematischen Zunahme ihrer Zahl. Doel van het project is maatregelen te nemen ter verbetering van de gezondheidstoestand van mensen die risico lopen op armoede en sociale uitsluiting uit de gemeente Korfant en omgeving: ouderen en gehandicapten. Het project voorziet in dagelijkse gezondheidsdiensten in de thuisomgeving en in het Opole Revalidatiecentrum. Er zal een huur van revalidatie apparatuur, evenals een educatieve activiteit: opleiding voor feitelijke zorgverleners op het gebied van medische zorg en voor medisch personeel om de competentie te verbeteren en de kwaliteit van de verleende diensten te verbeteren. In het project wordt de voorkeur gegeven aan mensen met een medische indicatie, hetzij direct, hetzij tot een half jaar na de ziekenhuisopname, die voldoen aan de voorwaarden voor aanwerving. 10 plaatsen van zorg worden gecreëerd in de dagelijkse vorm van verblijf en maximaal 6 patiënten per dag worden pendelen door OCR-personeel (een vertrekgroep zal worden gestart). Het project reageert op de waargenomen sociale problemen: — het versnelde verouderingsproces (toename van het aantal eenpersoonshuishoudens van alleenwonende ouderen met een moeilijke toegang tot gezondheidsdiensten) en het probleem van de verhoging van de economische lasten voor gezinnen, — de moeilijkheid of het onvermogen om zelfstandig basislevensactiviteiten uit te voeren (zoals aangegeven door de Barthel-schaal) en de behoefte aan verbeteringsmaatregelen voor mensen met een handicap en afhankelijke personen, met name op 65-jarige leeftijd, — de geringe mate van het voorzien in de behoeften van oudere bewoners in termen van woonplaats in de dagelijkse woningen, met een systematische toename van het aantal mensen. Als gevolg van het project zal de verpleging en medische bijstand, met inbegrip van revalidatie en fysiotherapie, ergotherapie, worden geïntensiveerd, wat leidt tot verbetering en verbetering van het welzijn van deze mensen. L'obiettivo del progetto è quello di adottare misure per migliorare lo stato di salute delle persone a rischio di povertà ed esclusione sociale dal comune di Korfant e dall'area circostante: anziani e disabili. Il progetto fornirà servizi sanitari nell'ambiente domestico e nel centro di riabilitazione di Opole in forma quotidiana. Ci sarà un noleggio di attrezzature di riabilitazione, così come un'attività educativa: formazione per i veri prestatori di assistenza nel settore dell'assistenza medica e per il personale medico, al fine di migliorare le competenze e la qualità dei servizi forniti. Nel progetto, le persone con un'indicazione medica saranno preferite direttamente o fino a mezzo anno dopo il ricovero, soddisfacendo le condizioni per il reclutamento. 10 posti di cura saranno creati in forma giornaliera di soggiorno e fino a 6 pazienti al giorno saranno pendolari da personale OCR (un gruppo di partenza sarà lanciato). Il progetto risponde ai problemi sociali percepiti: — il processo di invecchiamento accelerato (aumento del numero di famiglie monoparentali di persone anziane che vivono da sole con difficoltà di accesso ai servizi sanitari) e il problema dell'aumento dell'onere economico per le famiglie, — la difficoltà o l'incapacità di svolgere autonomamente attività vitali di base (come indicato dalla scala Barthel) e la necessità di misure di miglioramento per le persone con disabilità e le persone non autosufficienti, soprattutto all'età di 65 anni, — la piccola misura di garantire le esigenze dei residenti anziani in termini di luoghi di residenza nelle abitazioni quotidiane, con un aumento sistematico del loro numero. Come risultato del progetto, l'assistenza infermieristica e medica, compresa la riabilitazione e la fisioterapia, la terapia occupazionale, sarà intensificata, portando a migliorare e migliorare il benessere di queste persone. El proyecto proporcionará diariamente servicios de salud en el hogar y en el Centro de Rehabilitación de Opole. Como resultado del proyecto, se intensificará la asistencia de enfermería y médica, incluida la rehabilitación y fisioterapia, la terapia ocupacional, lo que conducirá a mejorar y mejorar el bienestar de estas personas.
