Projekt realizowany przez NZOZ PRAXIS spółka z.o.o. skierowany jest do: 70 osób niesamodzielnych (w tym co najmniej 75% osób powyżej 65 r.życia) zamieszkujących na terenie miasta Nowy Sącz lub gminy Chełmiec,których stan zdrowia nie pozwala na pozostawanie wyłącznie pod opieką podstawowej opieki zdrowotnej i ambulatoryjnej opieki specjalistycznej, a jednocześnie nie wymagających całodobowego nadzoru lekarskiego i pielęgniarskiego realizowanego w trybie stacjonarnym spełniających m.in. następujące warunki: 1) a) osoby bezpośrednio po przebytej hospitalizacji, których stan zdrowia wymaga wzmożonej opieki pielęgniarskiej, nadzoru nad terapią farmakologiczną, kompleksowych działań usprawniających i przeciwdziałających postępującej niesamodzielności oraz wsparcia i edukacji zdrowotnej, a także doradztwa w zakresie organizacji opieki i leczenia osoby niesamodzielnej. b) osoby, którym w okresie ostatnich 12 miesięcy udzielone zostały świadczenia zdrowotne z zakresu leczenia szpitalnego. 2) osoby wymagające,ze wzgl.na stan zdrowia,świadczeń pielęgnacyjnych,opiekuńczych i rehabilitacyjnych oraz kontynuacji leczenia,a niewymagający hospitalizacji w oddziale szpitalnym,którzy w ocenie skalą poziomu samodzielności (skala Barthel) otrzymają 40-65 punktów. 3) osoby mające prawo do świadczeń opieki zdrow.finans.ze środków publicznych. Cel projektu to: Deinstytucjonalizacja opieki nad osobami zależnymi (w tym osobami powyżej 65 roku życia) z terenu miasta Nowego Sącza oraz gminy Chełmiec poprzez utworzenie i prowadzenie Dziennego Domu Opieki Medycznej (DDOM). W ramach proj. utworzony i doposażony zostanie DDOM (deinstytucjonalizacja opieki nad osobami zależnymi) z 12 miejscami, realizowane będą świadczenia zdrowotne dla ogółem 70 pacjentów,w tym 75% osób powyżej 65 r.życia, prowadzone będą szkolenia dla pacjentów i ich rodzin/ opiekunów. Założono, że min. 80% osób korzystających z usług DDOM będzie pozytywni (Polish)

The project implemented by NZOZ PRAXIS Spółka z.o.o. is addressed to: 70 dependants (including at least 75 % of people over 65) living in the town of Nowy Sącz or the municipality of Chełmiec, whose state of health does not allow to remain solely under primary care and outpatient care, while at the same time not requiring round-the-clock medical and nursing supervision carried out on a stationary basis, fulfilling inter alia the following conditions: 1. (a) persons immediately after hospitalisation whose health condition requires increased nursing care, supervision of pharmacological therapy, comprehensive measures to improve and counteract progressive dependency and support and health education, as well as advice on the organisation of care and treatment of dependent persons. b) persons to whom health services in the field of hospital treatment have been granted during the last 12 months. 2) persons requiring, regardless of the state of health, care, care and rehabilitation benefits and continuation of treatment, and who do not require hospitalisation in the hospital ward, who will receive 40-65 points in the assessment of the level of self-reliance (Barthel scale). 3) Persons who have the right to health care services.financial.from public funds. The objective of the project is to: Deinstitutionalisation of care for dependents (including persons over the age of 65) from the area of Nowy Sącz and the municipality of Chełmiec through the establishment and running of the Daily Medical Care House (DDOM). Le projet mis en œuvre par NZOZ PRAXIS Sp. z.o.o. vise à: 70 personnes à charge (dont au moins 75 % des personnes âgées de plus de 65 ans) vivant à Nowy Sącz ou dans la commune de Chełmiec, dont l’état de santé ne leur permet pas de rester exclusivement sous le régime de soins de santé primaires et de soins spécialisés ambulatoires, sans exiger une surveillance médicale et infirmière 24 heures sur 24, effectuée à l’arrêt et répondant, entre autres, aux conditions suivantes: 1) a) les personnes immédiatement après l’hospitalisation, dont l’état de santé nécessite une intensification des soins infirmiers, une surveillance pharmacologique, des mesures globales d’amélioration et de lutte contre la dépendance progressive, ainsi que des services de soutien et d’éducation en matière de santé, ainsi que des conseils sur l’organisation des soins et des traitements pour les personnes dépendantes. b) les personnes qui ont reçu des services de santé au cours des douze derniers mois dans le domaine des soins hospitaliers. 2) les personnes qui ont besoin, en termes de santé, de soins, de soins, de réadaptation et de poursuite du traitement, et qui n’ont pas besoin d’hospitalisation dans le service hospitalier, qui, dans l’évaluation du niveau d’indépendance (échelle Barthel), recevront de 40 à 65 points. 3) les personnes ayant droit à des soins de santé et à des soins financiers provenant de fonds publics. Le projet mettra en place et modernisera le DDOM (désinstitutionnalisation des soins aux personnes à charge) de 12 places, fournissant des services de santé à 70 patients au total, dont 75 % des personnes de plus de 65 ans, une formation pour les patients et leurs familles/soignants. Das von NZOZ PRAXIS Sp. z.o.o. durchgeführte Projekt richtet sich an: 70 unterhaltsberechtigte Personen (darunter mindestens 75 % der Personen über 65 Jahre), die in Nowy Sącz oder der Gemeinde Chełmiec leben, deren Gesundheitszustand es ihnen nicht gestattet, ausschließlich im Rahmen der primären Gesundheitsversorgung und der ambulanten Spezialversorgung zu bleiben, ohne dass eine 24-Stunden-Gesundheits- und Pflegeaufsicht auf stationärer Basis durchgeführt wird, die unter anderem folgende Bedingungen erfüllt: 1) a) Personen unmittelbar nach dem Krankenhausaufenthalt, deren Gesundheitszustand eine verstärkte Pflege, pharmakologische Überwachung, umfassende Maßnahmen zur Verbesserung und Bekämpfung der fortschreitenden Abhängigkeit sowie Gesundheitsunterstützung und Bildung sowie Beratung bei der Organisation der Pflege und Behandlung von unterhaltsberechtigten Personen erfordert. b) Personen, die in den letzten 12 Monaten im Bereich der Krankenhausbehandlung Gesundheitsdienstleistungen erhalten haben. 2) Personen, die in Bezug auf Gesundheit, Pflege, Pflege und Rehabilitation und Fortsetzung der Behandlung benötigen und die keine Krankenhausaufenthalte in der Krankenhausstation erfordern, die bei der Beurteilung des Grads der Unabhängigkeit (die Barthel-Skala) 40-65 Punkte erhalten. 3) Personen, die Anspruch auf Gesundheits- und Finanzversorgung aus öffentlichen Mitteln haben. Mit dem Projekt soll das DDOM (Deinstitutionalisierung der Versorgung von Angehörigen) mit 12 Plätzen eingerichtet und nachgerüstet werden, wobei insgesamt 70 Patienten, darunter 75 % der über 65 Patienten, Gesundheitsdienstleistungen für Patienten und deren Familien/Betreuer anbieten. Het door NZOZ PRAXIS Sp. z.o.o. uitgevoerde project is gericht op: 70 personen ten laste (waarvan ten minste 75 % van de personen ouder dan 65 jaar) die in Nowy Sącz of in de gemeente Chełmiec wonen, wier gezondheidstoestand hen niet toestaat om uitsluitend onder de eerstelijnsgezondheidszorg en gespecialiseerde ambulante zorg te blijven, zonder dat 24 uur per dag medisch en verpleegkundig toezicht vereist is, dat onder meer voldoet aan de volgende voorwaarden: 1) a) personen onmiddellijk na ziekenhuisopname, wier gezondheidstoestand een intensievere verpleging vereist, farmacologisch toezicht, alomvattende maatregelen ter verbetering en bestrijding van de progressieve afhankelijkheid, gezondheidsondersteuning en onderwijs, alsmede advies over de organisatie van de zorg en de behandeling van afhankelijke personen; b) personen die in de laatste twaalf maanden gezondheidsdiensten hebben gekregen op het gebied van ziekenhuisbehandeling. 2) personen die, wat betreft gezondheid, zorg, zorg en revalidatie en voortzetting van de behandeling, en die geen ziekenhuisopname nodig hebben op de ziekenhuisafdeling, die bij de beoordeling van het niveau van onafhankelijkheid (de Barthel-schaal) 40-65 punten zullen ontvangen. 3) personen die recht hebben op gezondheidszorg en financiële zorg uit overheidsmiddelen. Il progetto attuato da NZOZ PRAXIS Sp. z.o.o. mira a: 70 persone a carico (incluso almeno il 75 % delle persone di età superiore a 65 anni) che vivono in Nowy Sącz o nel comune di Chełmiec, il cui stato di salute non consente loro di rimanere esclusivamente sotto l'assistenza sanitaria di base e le cure specialistiche ambulatoriali, senza richiedere un controllo medico e infermieristico 24 ore su 24, effettuato su base stazionaria, rispondendo, tra l'altro, alle seguenti condizioni: 1) a) le persone immediatamente dopo il ricovero, il cui stato di salute richiede un'intensificazione delle cure infermieristiche, la supervisione farmacologica, le misure globali per migliorare e combattere la dipendenza progressiva, il sostegno sanitario e l'istruzione, nonché la consulenza sull'organizzazione delle cure e delle cure per le persone non autosufficienti. b) le persone che hanno ricevuto servizi sanitari negli ultimi 12 mesi nel settore delle cure ospedaliere. 2) le persone che necessitano, in termini di salute, cure, cure e riabilitazione e continuazione delle cure, e che non necessitano di ricovero ospedaliero nel reparto ospedaliero, che nella valutazione del livello di indipendenza (scala Barthel) riceveranno 40-65 punti. 3) persone aventi diritto all'assistenza sanitaria e finanziaria con fondi pubblici.
