Celem głównym p w okresie 01.12.2020-31.06.2023r jest wzrost dostępności usług społ na ter Gminy Jerzmanowa o 21miejsc, dla 61os. zagrożonych ubóstwem lub wyklucz społ korzystających z usług społecznych poprzez:-zwiększenie o 1miejsce(1os) lb. świad specjalistycznych usług opiek w miejscu zamieszkania dla 1(1M) osób niesamodzielnych (w tym niepełnosprawnych);-zwiększenie o 20lb miejsc usług opiekuńczych świad w formie stacjonarnej –w placówce zapewniającej dzienną opiekę nad osobami niesamodzielnymi-utworzenie KlubuSeniora dla 60(45K,15M) os niesamodzielnych, starszych;-zwiększenie mobilności i bezpieczeństwa 20(15K,5M)os. niesamodzielnych poprzez utworzenie wypożyczalni sprzętu wspomagająco–pielęgnacyjnego;Ponadto, uczestnikiem p. będzie 1osoba(1K), która zostanie przeszkolona w zakresie pełnienia funkcji opiekuna KS, tak by możliwe było zapewnienie tej usługi na terenie gminy. W ramach p. założono działania:-utworzenie KS w na ter. G.J., a w nim realizacja zajęć, warsztatów, wyjazdy kulturalne;-zatrudnienie osoby świadczącej specjalistyczne usługi w miejscu zamieszkania;-utworzenie wypożyczalni sprzętu rehabilitacyjno-wspomagającego. GRUPA DOCELOWA to 61osób:-60os(45K,15M) niesamodzielnych, które otrzymają wsparcie w postaci usług opiekuńczych w Klubie Seniora;-1os(1M) z niepełnosprawnościami, chłopiec poniżej 16r.ż., który otrzyma wsparcie w postaci specjalistycznych usług opiek w miejscu zamieszkania, w formie rehabilitacji fizycznej i usprawniania zaburzonych funkcji organizmu w zakresie nieobjętym przepisami ust z 27.08.2004 o świadczeniach opieki zdrowotnej, której zakres okr zgodnie z zaleceniami lekarskimi lub specjalisty z zakresu rehabilitacji ruchowej lub fizjoterapii;P. real jest w odp na zdiagnozowane potrzeby w zakresie wsparcia osób niesamodzielnych, starszych i niepełnosprawnych na terenie G.J. Nakierowany jest na stworzenie miejsc usług opiekuńczych, które ze względu na niewystarczające środki fin i kadrowe nie były w wystarczającym stopniu real. (Polish)
