Przedmiotem projektu jest Modernizacja dworca kolejowego w Choszcznie jako platformy organizacji społecznych w tym utworzenie Centrum Obsługi Osób Zagrożonych Wykluczeniem Społecznym, który będzie pełnił strategiczne znaczenie w procesie aktywizacji mieszkańców gm. Choszczno. Zakres rzeczowy/etapy: 1.Prace przygotowawcze: proj.budowlany, kosztorysy,audyt energetyczny,SW,pozwolenie na budowę 2.Wyłonienie wykonawców:inspektor nadzoru inwestorskiego,wykonawca prac budowlanych,dostawca wyposażenia,promocja 3.Rewitalizacja dworca-etap I-branża budowlana,branża elektryczna,instalacja sanitarna wewnętrzna 4.Rewitalizacja dworca – etap II –branża budowlana, kanalizacja sanitarna z przyłączem, kanalizacja deszczowa z przyłączem, instalacja wodociągowa z przyłączem, przyłącze gazu pomiar, Instalacje sanitarne wewnętrzne, branża elektryczna, droga p.poż, zakup wyposażenia centrum (w tym: wyposażenie sali szkoleniowej,biura dla prac.socjalnychm.in.:biurka,krzesła, rzutnik, stoły, krzesła itp.)-utworzenie/zagospodarowanie pomieszczeń biurowych na cele społeczne w tym Miejsko-Gminnego Ośrodka Pomocy Społecznej, Centrum Obsługi Osób Zagrożonych Wykluczeniem Społecznym, Biurem Obsługi Szkół Samorządowym, Choszczeński Zarząd Nieruchomości Komunalnych,stowarzyszenia działające na rzecz społeczności lokalnej 5.Inspektor nadzoru inwestorskiego 6.Promocja projektu Opis planowanych prac patrz pkt.3.1.2/4.1.3.2 SW Sposób wdrażania – za realizację projektu odpowiedzialny będzie Wnioskodawca. Planuje się wybór wykonawców zgodnie z PZP, wnioskodawca dysponuje zasobami niezbędnymi do wdrożenia projektu –patrz pkt.3.2.2 SW. Zakończenie i oddanie obiektu do użytku:odbiór końcowy przez inwestora, zawiadomienie o zakończeniu robót do Powiatowego Inspektora Nadzoru Budowlanego. The subject of the project is the modernisation of the railway station in Choszowo as a platform for social organisations, including the creation of a Centre for the Services of Persons Threat of Social Exclusion, which will be of strategic importance in the process of activation of residents of Gm. Choszczno. Material scope/stages: 1.Preparatory work: construction project, cost estimates, energy audit, SW, construction permit 2.Exhibition of contractors: inspector of investor supervision, contractor of construction works, supplier of equipment, promotion 3.Regeneration of station-stage I-stage I-construction industry, electrical sector, internal sanitary installation 4.Regeneration of the station – stage II – construction industry, sanitary drainage with connection, rain drainage, water supply, connection equipment of the training room,office for social works, e.g.:office, chairs, projector, tables, chairs, etc.)-creation/use of office space for social purposes including the Local-Gmina Social Assistance Centre, Centre for the Service of People Threated by the Social Exclusion, the implementation of local schools will be carried out by the Local Government School, Choszczeński Management of Municipal Property, Societal Property Management, Associations for Social Exclusion. It is planned to select contractors in accordance with the PHP, the applicant has the necessary resources to implement the project – see point.3.2.2 SW. Completion and commissioning of the facility: final reception by the investor, notification of completion of works to the District Inspector of Construction Supervision.
