Projekt skierowany jest do 8 (6K,2 M) mieszkańców gminy Człopa, będących osobami zagrożonymi wykluczeniem społecznym i/lub ubóstwem. W ramach projektu zaplanowano realizację następujących form wsparcia: - utworzenie i wsparcie dla funkcjonowania 2 mieszkań chronionych - treningowych oferujących 4 miejsca; - świadczenie usług społecznych w lokalnej społeczności dla osób korzystających z mieszkań chronionych powstałych w ramach projektu. Grupa docelowa: Projekty jest skierowany bezpośrednio do następującej grupy odbiorców: - osoby zagrożone ubóstwem i/lub wykluczeniem społecznym, - osoby z niepełnosprawnościami i ich rodziny; Wsparciem w ramach projektu objętych zostanie 8 osób (6K,2M) kwalifikujących się do wsparcia. Zadania zaplanowane w projekcie: - stworzenie (adaptacja i wyposażenie) 2 mieszkań chronionych oferujących 4 miejsca wsparcia - zapewnienie funkcjonowania 2 mieszkań chronionych i kompleksowane wsparcie osób w nich przebywających. Rezultaty: Liczba utworzonych w programie miejsc świadczenia usług w mieszkaniach wspomaganych i chronionych istniejących po zakończeniu projektu: 4. W przypadku mieszkań chronionych, zapewnia się, że jest stosowany standard (m.in. rodzaj i zakres wsparcia świadczonego w mieszkaniu chronionym oraz standard lokalu przeznaczonego na mieszkanie chronione) dotyczący tej formy pomocy wynikający z ustawy z dnia 12 marca 2014 r. o pomocy społecznej i rozporządzenia Ministra Pracy i Polityki Społecznej z dnia 14 marca 2012 r. w sprawie mieszkań chronionych. Usługi w ramach mieszkań wspomaganych wykonują również specjaliści w danej dziedzinie (opiekun, asystent) posiadający stosowne uprawnienia i kwalifikacje, zatrudniani do realizowania określonych rodzajów usług społecznych zg z Załącznikiem nr 7.19 Minimalne wymagania świadczenia usług społecznych w społeczności lokalnej. The project plans to implement the following forms of support: — creation and support for the functioning of 2 protected apartments – training apartments offering 4 seats; — the provision of social services in the local community for those benefiting from protected housing created within the framework of the project. Target group: The projects are addressed directly to the following group of recipients: — people at risk of poverty and/or social exclusion, – persons with disabilities and their families; The project will support 8 people (6K,2M) eligible for support. Tasks planned in the project: — creation (adaptation and equipment) of 2 protected dwellings offering 4 support places – ensuring the functioning of 2 protected dwellings and complex support for those staying in them. Results: Number of places established in the programme to provide services in assisted and protected dwellings existing after the completion of the project: 4. In the case of protected dwellings, it is ensured that the standard (e.g. the type and scope of support provided in a protected housing and the standard of housing intended for protected housing) is applied to this form of assistance resulting from the Act of 12 March 2014 on Social Assistance and the Regulation of the Minister of Labour and Social Policy of 14 March 2012 on protected dwellings. Assisted housing services are also provided by specialists in a given field (career, assistant) with appropriate qualifications and qualifications, employed to provide certain types of social services according to Annex No 7.19 Minimum requirements for the provision of social services in the local community. Le projet s’adresse à 8 résidents (6K,2 M) de la municipalité du député qui sont exposés au risque d’exclusion sociale et/ou de pauvreté. Dans le cadre du projet, les formes de soutien suivantes sont prévues: — création et soutien à l’exploitation de 2 appartements protégés — unités de formation offrant 4 places; — fourniture de services sociaux dans la communauté locale aux personnes utilisant des logements protégés créés dans le cadre du projet. Groupe cible: Les projets s’adressent directement au groupe de bénéficiaires suivant: — les personnes exposées au risque de pauvreté et/ou d’exclusion sociale, — les personnes handicapées et leur famille; Le projet soutiendra 8 personnes (6K,2M) éligibles à l’aide. Tâches prévues dans le projet: — la création (adaptation et équipement) de 2 appartements protégés offrant 4 places de soutien — assurant le fonctionnement de 2 appartements protégés et un soutien complexe pour ceux qui y séjournent. Résultats: Nombre de logements assistés et protégés créés dans le cadre du programme après l’achèvement du projet: 4. Dans le cas des appartements protégés, il est garanti que la norme (y compris le type et l’étendue de l’aide fournie dans le logement protégé et la norme des locaux destinés à un logement protégé) est utilisée pour cette forme d’assistance, conformément à la loi du 12 mars 2014 relative à l’assistance sociale et au règlement du ministre du travail et de la politique sociale du 14 mars 2012 sur le logement protégé. Les services d’aide au logement sont également fournis par des spécialistes dans un domaine donné (soignant, assistant) possédant les qualifications et les qualifications appropriées, employés pour fournir certains types de services sociaux conformément à l’annexe 7.19 Prescriptions minimales pour la fourniture de services sociaux dans la communauté locale. Das Projekt richtet sich an 8 (6K,2 Mio) Einwohner der Gemeinde des Mitglieds, die von sozialer Ausgrenzung und/oder Armut bedroht sind. Im Rahmen des Projekts sind folgende Förderformen geplant: — Schaffung und Unterstützung für den Betrieb von 2 geschützten Wohnungen – Schulungseinheiten mit 4 Plätzen; — die Bereitstellung sozialer Dienstleistungen in der lokalen Gemeinschaft für Personen, die im Rahmen des Projekts einen geschützten Wohnraum nutzen. Zielgruppe: Die Projekte richten sich direkt an folgende Empfängergruppe: Personen, die von Armut und/oder sozialer Ausgrenzung bedroht sind, – Menschen mit Behinderungen und ihre Familien; Das Projekt unterstützt 8 Personen (6K,2M), die für eine Förderung in Frage kommen. Im Projekt geplante Aufgaben: — die Schaffung (Anpassung und Ausrüstung) von 2 geschützten Wohnungen, die 4 Plätze der Unterstützung bieten – Gewährleistung des Funktionierens von 2 geschützten Wohnungen und komplexe Unterstützung für diejenigen, die in ihnen bleiben. Ergebnisse: Anzahl der geförderten und geschützten Wohnungen, die nach Abschluss des Projekts im Rahmen des Programms eingerichtet wurden: 4. Bei geschützten Wohnungen wird sichergestellt, dass die Norm (einschließlich der Art und des Umfangs der Unterstützung in der geschützten Wohnung und des Standards der Räumlichkeiten für geschützte Wohnungen) für diese Form der Unterstützung verwendet wird, wie sie im Gesetz vom 12. März 2014 über Sozialhilfe und in der Verordnung des Ministers für Arbeit und Sozialpolitik vom 14. März 2012 über den Schutz von Wohnraum festgelegt ist. Unterstützte Wohnungsdienstleistungen werden auch von Fachleuten in einem bestimmten Bereich (Betreuer, Assistenten) mit entsprechenden Qualifikationen und Qualifikationen erbracht, die für bestimmte Arten von Sozialdienstleistungen gemäß Anhang 7.19 Mindestanforderungen für die Erbringung von Sozialdienstleistungen in der lokalen Gemeinschaft eingesetzt werden. Het project is gericht op 8 (6K,2 miljoen) inwoners van de gemeente van het lid die het risico lopen op sociale uitsluiting en/of armoede. In het kader van het project zijn de volgende vormen van steun gepland: — oprichting en ondersteuning van de exploitatie van 2 beschermde appartementen — opleidingseenheden met 4 plaatsen; — verlening van sociale diensten in de lokale gemeenschap voor mensen die gebruik maken van beschermde huisvesting die in het kader van het project is gecreëerd. Doelgroep: De projecten zijn rechtstreeks gericht tot de volgende groep begunstigden: personen die het risico lopen op armoede en/of sociale uitsluiting, — personen met een handicap en hun gezinnen; Het project zal 8 personen (6K,2M) ondersteunen die in aanmerking komen voor steun. In het project geplande taken: — de oprichting (aanpassing en uitrusting) van 2 beschermde appartementen met 4 plaatsen van ondersteuning — zorgen voor de werking van 2 beschermde appartementen en complexe ondersteuning voor degenen die er verblijven. Resultaten: Aantal ondersteunde en beschermde woningen dat na voltooiing van het project in het programma is gecreëerd: 4. In het geval van beschermde appartementen wordt gewaarborgd dat voor deze vorm van bijstand gebruik wordt gemaakt van de norm (met inbegrip van het type en de omvang van de steun die wordt verleend in de beschermde woning en de standaard van gebouwen bestemd voor beschermde huisvesting), zoals vastgelegd in de Wet van 12 maart 2014 inzake sociale bijstand en het besluit van de minister van Arbeid en Sociaal Beleid van 14 maart 2012 inzake beschermde huisvesting. Ondersteunende huisvestingsdiensten worden ook verleend door specialisten op een bepaald gebied (verzorger, assistent) met passende kwalificaties en kwalificaties, die werkzaam zijn om bepaalde soorten sociale diensten te verlenen overeenkomstig bijlage 7.19 Minimumvoorschriften voor het verlenen van sociale diensten in de plaatselijke gemeenschap. Il progetto è rivolto a 8 (6K,2 M) residenti del Comune del deputato che sono a rischio di esclusione sociale e/o povertà. Nell'ambito del progetto sono previste le seguenti forme di sostegno: — creazione e supporto per il funzionamento di 2 appartamenti protetti — unità di formazione che offrono 4 posti; — fornitura di servizi sociali nella comunità locale per le persone che utilizzano alloggi protetti creati nell'ambito del progetto. Gruppo destinatario: I progetti sono indirizzati direttamente al seguente gruppo di destinatari: — le persone a rischio di povertà e/o di esclusione sociale, — le persone con disabilità e le loro famiglie; Il progetto sosterrà 8 persone (6K,2M) ammissibili al sostegno. Compiti previsti nel progetto: — la creazione (adattamento e attrezzature) di 2 appartamenti protetti che offrono 4 posti di supporto — garantendo il funzionamento di 2 appartamenti protetti e un supporto complesso per coloro che vi soggiornano. Risultati: Numero di abitazioni assistite e protette create nell'ambito del programma dopo il completamento del progetto: 4. Nel caso degli appartamenti protetti, si garantisce che la norma (compresi il tipo e la portata del sostegno fornito nell'abitazione protetta e la norma dei locali destinati all'edilizia abitativa protetta) sia utilizzata per questa forma di assistenza, come previsto dalla legge del 12 marzo 2014 sull'assistenza sociale e dal regolamento del Ministro del lavoro e delle politiche sociali del 14 marzo 2012 sull'edilizia abitativa protetta. I servizi di alloggio assistiti sono forniti anche da specialisti in un determinato settore (assistente, assistente) con qualifiche e qualifiche adeguate, impiegati per fornire determinati tipi di servizi sociali conformemente all'allegato 7.19 Requisiti minimi per la prestazione di servizi sociali nella comunità locale.
